Job Purpose:
- The RN Case Manager at HOMECARE COMPANY OF TENNESSEE LLC is responsible for coordinating and managing patient care in a homecare setting. The role involves assessing patient needs, developing care plans, and ensuring the delivery of high-quality healthcare services. The RN Case Manager acts as a liaison between patients, families, and healthcare providers to facilitate effective communication and optimize patient outcomes.
Key Responsibilities:
- Conduct comprehensive assessments of patients to determine their healthcare needs and develop individualized care plans.
- Coordinate and manage patient care by collaborating with healthcare providers, patients, and families to ensure continuity of care.
- Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
- Educate patients and their families on healthcare needs, treatment plans, and available resources to promote self-management and independence.
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory standards and company policies.
- Advocate for patients by identifying and addressing any barriers to care and facilitating access to necessary healthcare services.
- Participate in interdisciplinary team meetings to discuss patient care plans and collaborate on best practices.
- Provide leadership and guidance to other healthcare staff to ensure the delivery of high-quality patient care.
- Stay informed about the latest developments in home healthcare and integrate evidence-based practices into patient care.
Required Education:
- Preferred Bachelor of Science in Nursing (BSN) from an accredited institution.
- Active and unrestricted Registered Nurse (RN) license in the state of Tennessee.
Required Experience:
- Minimum of 2 years of clinical nursing experience, preferably in a home care or case management setting.
- Proven experience in developing care plans and coordinating patient care across multi-disciplinary teams.
- Experience with electronic health records (EHR) and case management software.
Required Skills and Abilities:
- Strong assessment and critical thinking skills to evaluate patient needs and develop effective care plans.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare professionals.
- Ability to work independently and manage a caseload efficiently while maintaining high standards of care.
- Proficient in time management and organizational skills to prioritize tasks and meet deadlines.
- Knowledge of healthcare regulations and compliance standards related to home care services.
- Compassionate and empathetic approach to patient care, with a focus on improving patient outcomes and quality of life.
