Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess extensive clinical experience across various specialties, including long-term care, acute care, emergency medicine, and geriatrics. This role involves providing high-quality patient care in diverse settings such as hospitals, nursing homes, assisted living facilities, and home care environments. The LPN will utilize their expertise in medical documentation, patient assessment, medication administration, and specialized procedures to support patient recovery and well-being. Familiarity with EMR/EHR systems like Epic, Cerner, and Athenahealth is essential to ensure accurate record-keeping and compliance with HIPAA standards.
Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation.
- Administer medications and injections accurately following physician orders while adhering to sterile technique and aseptic protocols.
- Manage IV infusions, catheter care, tube feeding, and ventilator support as needed for various patient populations.
- Assist with diagnostic evaluations such as blood draws (phlebotomy), specimen collection/processing, and imaging procedures.
- Provide specialized care for patients with complex conditions including dementia, Alzheimer's disease, traumatic injuries (Level I & II Trauma Centers), infectious diseases, and post-acute recovery.
- Support patients in pain management strategies including acute pain control and chronic condition management.
- Collaborate with multidisciplinary teams in settings like ICU, PICU, neonatal units, hospice care, and outpatient clinics to develop comprehensive discharge plans and coordinate ongoing care.
- Maintain accurate medical records using EMR/EHR systems like eClinicalWorks or Cerner while ensuring compliance with HIPAA regulations.
- Educate patients and families on health coaching topics such as medication adherence, nutrition, disease management (e.g., diabetes), and post-discharge instructions.
- Ensure infection control practices are followed rigorously to prevent healthcare-associated infections.
- Perform advanced procedures such as suturing, spinal taps, cardiac catheterization assistance, or dermal fillers when applicable.
Requirements
- Valid Licensed Practical Nurse license in the state of practice with current certification.
- Proven experience in diverse clinical settings including hospitals (Emergency Room experience preferred), nursing homes (Geriatrics), assisted living facilities, or home & community care environments.
- Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation techniques.
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for documentation purposes.
- Experience administering injections (including IV insertion), phlebotomy/venipuncture skills required.
- Ability to work independently or as part of a team in fast-paced environments like trauma centers or urgent care clinics.
- Knowledge of infection control protocols and aseptic techniques necessary for sterile procedures like catheterization or dialysis assistance.
- Excellent patient assessment skills with a focus on behavioral health issues such as dementia or mental health conditions related to neurodegenerative diseases.
- Critical thinking skills combined with compassionate patient-centered care approach for all age groups from neonatal to geriatric populations—including pediatric (Toddler & Neonatal) care when applicable.
- Must pass background checks and drug screening prior to employment.
