Job Summary
We are seeking a dedicated and compassionate Full-Time Home Health Aide (HHA) / Certified Nursing Assistant (CNA) to provide exceptional patient care in a variety of settings. The ideal candidate will have experience working with diverse populations, including seniors, individuals with developmental disabilities, and those requiring specialized dementia or Alzheimer's care. This role involves assisting patients with daily activities, monitoring health status, and ensuring a safe and supportive environment. The position offers an opportunity to make a meaningful difference in patients' lives while working within a professional healthcare team.

Responsibilities

  • Provide direct patient care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
  • Monitor and record vital signs accurately using EMR/EHR systems
  • Assist with medication administration following prescribed protocols and HIPAA regulations
  • Support patients with dementia, Alzheimer's, memory care, hospice, and long-term care needs
  • Operate medical equipment such as Hoyer lifts and catheter care devices safely and effectively
  • Document patient information thoroughly in medical records and electronic systems
  • Assist with meal preparation and light housekeeping tasks including laundry and cleaning to maintain a safe environment
  • Support patients during transfers using appropriate techniques and equipment
  • Observe patient conditions closely, report changes promptly to healthcare professionals, and maintain patient safety at all times

Skills

  • Proven experience in home health or long-term care environments, including assisted living or nursing homes
  • Strong knowledge of medical terminology, medication administration, and medical documentation standards
  • Skilled in caring for patients with dementia, Alzheimer's, developmental disabilities, or those requiring hospice or memory care services
  • Ability to operate EMR/EHR systems efficiently for documentation purposes
  • CPR certification required; additional certifications such as HIPAA compliance preferred
  • Experience working with people with disabilities or in community-based care settings is highly desirable
  • Compassionate caregiver with excellent communication skills and patience when working with diverse patient populations
  • Ability to work independently while following healthcare protocols and safety guidelines
