Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
  • Administer medications and injections accurately following established protocols while monitoring for adverse reactions.
  • Assist with medical procedures such as IV infusions, catheterizations, wound suturing, and sterile specimen collection.
  • Manage patient documentation diligently using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, or Athenahealth to ensure precise medical records.
  • Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or memory impairments through tailored interventions.
  • Support acute pain management strategies and participate in discharge planning to ensure seamless transitions of care.
  • Conduct patient triage in urgent care or emergency settings, prioritizing care based on severity and urgency.
  • Collaborate with multidisciplinary teams in diverse environments—including ICU, PICU (Pediatric Intensive Care Unit), neonatal units, hospice settings, and rehabilitation centers—to deliver comprehensive patient-centered care.
  • Educate patients and their families on health coaching topics such as medication management, nutrition, chronic disease management, and post-discharge instructions.
  • Adhere strictly to HIPAA regulations to protect patient privacy and confidentiality at all times.
  • Assist with specialized procedures like cardiac catheterization prep, airway management, ventilator support, or gastrostomy tube feeding when required.
  • Participate in infection control practices and maintain aseptic techniques during all clinical procedures to ensure safety standards are met.

Skills

  • Proven experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with ICD-9/ICD-10 coding for accurate documentation.
  • Critical care experience in settings like Level I Trauma Centers or Level II Trauma Centers is highly valued.
  • Extensive background in medical-surgical nursing, geriatrics—including Alzheimer's and dementia care—and long-term or sub-acute care environments.
  • Skilled in performing phlebotomy (venipuncture), administering injections, catheter care, tube feeding management, and basic life support (BLS).
  • Knowledge of physiology, anatomy, pharmacology related to pain management and medication administration.
  • Experience working with diverse populations including pediatrics (neonatal ventilators), adults in emergency medicine or urgent care settings, and individuals with developmental disabilities or behavioral health needs.
  • Ability to handle complex cases involving infectious disease control or hospice/end-of-life care with compassion and professionalism.
  • Strong communication skills for effective patient assessment and education; capable of working collaboratively within multidisciplinary teams.
  • Familiarity with medical imaging procedures or diagnostic evaluations is a plus.
