We are seeking a compassionate and experienced Dialysis Master Social Worker to join our healthcare team. Responsibilities
- Conduct thorough patient assessments to identify psychosocial needs, barriers to treatment adherence, and support requirements.
- Provide individual and group counseling sessions utilizing techniques such as behavioral therapy, motivational interviewing, and clinical counseling tailored to diverse patient populations.
- Assist patients and their families with coping strategies related to chronic illness management, end-of-life care, addiction recovery, or developmental disabilities.
- Coordinate with multidisciplinary teams to develop personalized care plans that incorporate social work best practices and medical documentation standards.
- Manage casework involving complex issues such as hospice care, Alzheimer's disease, dementia, autism spectrum disorders, PTSD, and geriatrics.
- Ensure compliance with HIPAA regulations by maintaining accurate medical records and documentation within EMR systems.
- Facilitate crisis intervention and provide immediate support during emergencies or behavioral health crises.
- Collaborate with community resources to connect patients with home & community care services, assisted living options, or outpatient programs as needed.
- Participate in clinical research projects related to behavioral health or social work interventions in healthcare settings.
- Maintain ongoing professional development through training on ICD coding (ICD-9/10), CPT coding, and current best practices in mental health counseling and case management.
Requirements
- Master's degree in Social Work (MSW) or related field from an accredited institution.
- Valid licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent preferred.
- Proven experience in hospital settings, inpatient units, nursing homes, outpatient clinics, or home & community care environments.
- Extensive knowledge of medical terminology, medical records management, HIPAA compliance, and EMR systems.
- Demonstrated expertise in child & family counseling, behavioral health therapies (including cognitive behavioral therapy), crisis management, and working with diverse populations such as seniors, children, individuals with developmental disabilities or autism spectrum disorder.
- Familiarity with diagnostic evaluation tools (ICD-9/10), CPT coding for billing purposes, and clinical research methodologies is advantageous.
- Strong communication skills with the ability to perform intake assessments effectively and collaborate within multidisciplinary teams.
- Ability to handle sensitive situations professionally while maintaining confidentiality and adhering to ethical standards. This role is integral to delivering holistic patient-centered care through expert social work practices tailored for dialysis patients with complex health conditions across various healthcare settings.
