Outreach Advocate (Career Path to Case Management)
Location: Washington State (Applicants must reside in Washington to be considered)
Full-Time | Hybrid/Remote (Outreach Phase)
Future Role Requires 90% Travel Within County of Residence
Position Overview: Outreach Advocate (6-Month Developmental Role)
This position begins as a Full-time Outreach Advocate for approximately six (6) months (dictated by Talent Acquisition and business need), with the intended transition into a Case Manager role, contingent upon performance, compliance, and business need.
HOURLY PAY: $24 per hour
As an Outreach Advocate, you will serve as the front line of engagement for our Health Home Program:
- Managing referrals
- Conduct Structure Outreach
- Produce accurate, compliant and timely documentation.
This role is highly structured, metric-driven, and fast-paced.
Following successful completion of the Outreach Advocate phase, this role transitions into a Care Coordinator position, which includes:
- 90% field-based travel within the county you reside in
- Direct, face-to-face member engagement
- Community-based care coordination and case management
What We're Looking For
Required
-Must reside in Washington State
-Must possess valid driver's license & Auto Insurance
-Associate's degree preferred
-Strong organizational skills and attention to detail
-Comfort working in structured, metrics-driven environments
-Comfortable juggling multiple platforms simultaneously
-Proficiency in Microsoft Office (Word, Excel, Outlook, Teams)
Preferred
⭐ Call Center experience
⭐ Sales background and/or medical/healthcare background
⭐ Experience in customer engagement, outreach, or member services
⭐ Bilingual or bicultural candidates encouraged to apply
Work Environment & Requirements
Ability to be on the phone for extended periods
Comfortable navigating multiple electronic systems
Willingness to meet clearly defined productivity metrics
Ability to meet future travel requirements for Case Management/Care Coordinator Role

Key Responsibilities – Outreach Advocate Phase
Referral Management & Eligibility Verification
- Receive referrals from Washington State Medicaid Managed Care Organizations
- Verify eligibility through ProviderOne and Lead Organization portals
- Conduct required state and national registry screenings
Client Outreach & Engagement
- Initiate timely outreach with assigned referrals
- Complete multiple contact attempts per referral in accordance with program standards
- Support enrollment into the Health Home Program
Client Matching & Scheduling Support
- Utilize geographic tools and system platforms to assign members to Care Coordinators
- Support efficient scheduling and field optimization
Documentation & Compliance
- Accurately document all outreach activity in OneHAP and Lead Organization systems
- Meet documentation quality, timeliness, and compliance standards
Care Team Collaboration
- Maintain consistent communication with Care Coordinators, Leads, and internal departments
- Support follow-up, rescheduling, and member inquiries
Performance Metrics
- Meet daily, weekly, and monthly productivity and outreach benchmarks
- Participate in audits, coaching, and performance reviews
Transition to Case Manager Role (Post 6 Months)
Upon transition, the role will include:
- 90% travel within the county you reside in
- Home visits and community-based member engagement
- Health Action Plan development and follow-up
- Complex care coordination and resource navigation
- Continued performance metrics, audits, and compliance oversight
