*Job Summary*
We are seeking a licensed and compassionate *Physical Therapist (PT)* to join our healthcare team in a skilled nursing facility setting. The Physical Therapist will evaluate and treat patients, develop and implement individualized treatment plans, and collaborate closely with an interdisciplinary care team to promote optimal patient outcomes. This role requires strong clinical judgment, effective communication skills, and a commitment to regulatory compliance and high-quality, patient-centered care. The ideal candidate will be experienced in geriatrics and skilled nursing, demonstrate a team-oriented approach, and be dedicated to improving mobility, function, and quality of life for our residents.
*Responsibilities*
* Evaluate patients based on physician referrals and develop effective, goal-oriented treatment plans with measurable functional outcomes.
* Treat patients according to established plans of care and adjust interventions based on patient progress and clinical judgment.
* Supervise Physical Therapist Assistants (PTAs), provisional/temporary licensees, and therapy students in accordance with state practice acts and reimbursement regulations.
* Maintain accurate, timely clinical documentation and daily treatment charges in compliance with Medicare and other payer requirements.
* Educate patients, families, and nursing staff on restorative techniques, home programs, and safety considerations.
* Participate in discharge planning, interdisciplinary team meetings, care conferences, utilization review meetings, and facility performance improvement (QAPI) initiatives.
* Communicate effectively with the Director of Rehab, physicians, nursing staff, and other team members regarding patient progress, concerns, and care plans.
* Conduct and participate in in-service education and ongoing training for staff.
* Assist with department organization, including equipment maintenance, securing durable medical/adaptive equipment, and maintaining a clean treatment area.
* Float or travel between facilities as needed based on patient care needs and supervisor direction.
* Perform therapy services via Telehealth when applicable, following all state and federal guidelines.
* Perform other duties as assigned by the Director of Rehab, Interdisciplinary Team, or Administrator.
*Skills*
* Strong knowledge of physical therapy principles, anatomy, physiology, and medical terminology.
* Demonstrated understanding and application of Medicare and other reimbursement regulations.
* Ability to develop and implement functional, patient-centered treatment plans for geriatric and medically complex populations.
* Excellent documentation and organizational skills with attention to regulatory compliance.
* Strong interpersonal and communication skills to work effectively with patients, families, and multidisciplinary teams.
* Positive, team-oriented attitude with flexibility and professionalism in a fast-paced healthcare environment.
* Ability to supervise and mentor PTAs and therapy students effectively.
*Qualifications*
*Education/Licensure*
* Graduate of an accredited college or university with a valid *Virginia Physical Therapist license* or eligibility to obtain a Virginia license.
*Physical Requirements*
* Ability to walk, stand, sit, bend, stoop, push, pull, and lift intermittently throughout the workday.
* Ability to lift patients, medical equipment, and supplies up to 50 lbs.
* Ability to communicate effectively in English, both verbally and in writing.
* Emotional stability and ability to manage the mental and emotional demands of the role.
* Ability to work with elderly, ill, disabled, emotionally upset, and occasionally hostile individuals.
* Valid driver’s license with the ability to travel between facilities as needed.
Job Type: Full-time
Pay: $44.00 - $60.00 per hour
