Über

We are seeking a dedicated Qualified Professional (QP) to join our Community Support Team (CST). The QP will provide direct and indirect support to adults with mental health and/or substance use challenges, ensuring that services are delivered in accordance with the Person-Centered Plan (PCP) and Medicaid guidelines. This is a billable service role.

Responsibilities:

  • Provide CST interventions in the community, home, or other appropriate settings.
  • Maintain and document billable service hours per agency and Medicaid requirements.
  • Complete assessments, service plans, progress notes, and other documentation accurately and timely.
  • Coordinate with clinical and medical staff, natural supports, and other service providers.
  • Support individuals in achieving treatment goals related to housing, employment, daily living, and wellness.
  • Participate in weekly team meetings, supervision, and case consultations.

Qualifications:

  • Bachelor's degree in Human Services or related field (Social Work, Psychology, Counseling, etc.).
  • Minimum of two years of post-degree experience working with the adult mental health or substance use population.
  • Knowledge of CST service definitions and North Carolina Medicaid requirements preferred.
  • Strong communication, organization, and teamwork skills.
  • Must have valid NC driver's license and reliable transportation.

Job Type: Full-time

Pay: $ $25.00 per hour
