We currently have an immediate need for nurses in Sandwich. Sandwich is our newest coverage areas. This position is for 2-5 days per week, seeing 4-6 patients in the morning. The Sandwich route is a great fit for nurses seeking a part-time schedule with the potential to expand visit volume as the route grows.
Responsibilities include:
* Driving to and visiting clients in their homes to provide one on one care, including administering and managing oral medications and injections, checking blood sugars and vital signs, observing, recording, and reporting changes in a client’s condition and needs to the clinical manager and/or physician, and educating clients and their caregivers.
* Executing, properly documenting, and following up with doctors’ orders.
* Initiating case conference review for complex, difficult, or high-risk cases.
* Completing documentation accurately and timely per agency protocol.
* Ensuring quality and accuracy of records.
* Communicating with other members of the client’s multidisciplinary team.
* Responding promptly to emails and phone calls from the care team.
* Participating in staff meetings, in-services, and other trainings per company policy and state regulations.
Requirements:
* Valid license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of practice.
* Knowledge of medical terminology and ability to understand and interpret physician orders.
* Ability to administer medications accurately and safely.
* Strong communication skills to effectively interact with patients, families, and healthcare team members.
* Ability to work well independently.
Additional responsibilities for RNs may include:
* Acting as case manager and assuming responsibility for coordinating client care for assigned caseload.
* Completing initial assessment and ongoing (Oasis) assessments.
* Supervising LPNs as appropriate.

Job Types: Per diem, Part-time
Pay: $30.00 - $45.00 per hour
Expected hours: 5 – 15 per week
