At the AIDS Center of Queens County (ACQC), our mission is to prevent HIV and STIs and promote health and wellness in the communities we serve. This is accomplished by providing health education and intervention services using a harm reduction, sex-positive approaches and evidence-based practices delivered in a friendly, respectful, and culturally sensitive environment. ACQC's programs focus on reducing new HIV/ STI/ HCV infections among women and young women of color by providing comprehensive health education, behavioral and biomedical interventions such as PrEP/PEP, and access to essential medical and prevention services.

Job Role

The Health Care Navigator/ Tester supports ACQC's mission by assisting with daily program operations, community outreach, health education, and coordination of prevention and care services for target populations.

Responsibilities

  • Assist with the day-to-day activities of the program.
  • Conduct on-going needs assessment including a review of characteristics, statistics, and data to develop or design services that are specific, sensitive, and culturally relevant to the target populations.
  • Conduct outreach in communities and target venues.
  • Develop and promote sexual health and HIV/STI prevention campaigns through social media, digital content, and virtual sessions to engage target populations.
  • Conduct community presentations and individual health education and risk reduction sessions.
  • Identify and address social determinants of health (SDOH) (e.g., health insurance, access to healthcare) that may impact participants' health outcomes and engagement in services.
  • Provide individual health education and risk reduction counseling.
  • Delivery of Counseling, Testing, & Referrals (CTR) – HIV/ STI/ HCV interventions.
  • Coordinate prevention services with agency and community partners—including HIV/STI care, PrEP/PEP, health insurance, medical care, and screenings (e.g., gynecological and breast health)—and provide linkage to care.
  • Follow up on referrals to ensure client engagement and service completion.
  • Maintain accurate and up-to-date client/peer chart records for program activities and CTR interventions.
  • Enter program data and submit weekly activity and progress reports.
  • Willing to be flexible with work schedule, which may include long hours, evenings, and weekends.
  • Other duties as assigned by the Program Supervisor or Director of Education & Prevention.

Skills, Qualifications, & Education

  • Bachelor's degree in social science or health related field and one-year experience: or, some college coursework and three to four years work or field related experience
  • Knowledge of HIV/AIDS and the communities most affected.
  • Understanding of STIs and the risk factors.
  • Fluency in one or more of targeted community languages and/or dialects preferred.
  • Must be culturally competent and able to work with individuals from non-traditional backgrounds in terms of gender identity and sexuality. Must be able to convey sensitivity in a non-judgmental manner.

The Health Education Specialist position is a full-time, non-exempt position, based in Richmond Hill/Jamaica, Queens near the LIRR and the E train.
