Care Coordinator is the person who supervises and directs home care aides that visit patients who need help in their homes with nursing, light tasks, and/or maintenance duties. The coordinator typically helps to ensure that properly credentialed care personnel are working with patients.
Job Responsibilities:
- Schedule and coordinate caregivers based on assessment and care plan information for new and existing clients.
- Maintain all scheduling of caregivers and respond to situations where scheduling conflicts, emergencies, and/or "call-outs" arise.
- Monitor hours of service delivered, quality of service delivery, client satisfaction and changes in client's functional status.
- Ability to take after hours calls and be on call for scheduling and Case Management
- Track and record in the internal database all instances of assignment refusals, call-outs, late arrivals, early departures, client/caregiver feedback, etc. Share information with the team.
- Any additional tasks that may be needed for top-notch client customer service.
Requirements:
- One years in a staffing/coordinator position
- Bilingual in both English and Spanish is a must
- Able to be on call for scheduling and Case Management
- Proven working experience in scheduling, case management or other related job field.
- Effective communication skills
- Excellent organizational and time management skills
- Problem solving skills and ability to multi-task
- Basic knowledge of Microsoft Word, Excel, and Google Sheets
- Knowledge of HHA Exchange.
Job Type: Full-time
