Your Future Team

Our San Diego office is currently seeking Case Managers for our THP-NMD program. My Brother's Keeper offers housing and supportive services to emancipated foster youth between the ages of 18 and 21. Our program is client-centered, trauma-informed, and upholds the highest standards of best practices.

Your Impact

Our program supports young adults aged 18-21 through a comprehensive approach, providing guidance in housing, employment, education, wellness, and other essential areas for a successful transition to independence.

Our Case Workers create a secure and supportive environment for participants as they move towards self-sufficiency. In this role, you will be involved in crisis intervention, case management, on-site community engagement, and the day-to-day operations of our programs at various times. You will use a strength-based, personalized approach to help our participants achieve their goals, while also ensuring a safe, clean, and nurturing living environment for all residents.

Your Skills and Duties as a Case Manager

Conduct intake assessments and develop individualized service plans.

Provide case management services, including housing support, life skills coaching, and educational/career guidance.

Facilitate access to community resources, such as healthcare, financial assistance, and employment programs.

Monitor progress and ensure compliance with THP-NMD regulations.

Advocate for the needs of clients and collaborate with social workers, service providers, and landlords.

Provide crisis intervention, situational support, and roommate mediation between participants

Keep well-written, concise case notes to include all pertinent information for each participant using County and State mandated tracking systems

Maintain well organized and accurate records and case files for all participants

Communicate effectively with Program Directors, co-workers, participants, families, and other service providers

Ensure residents maintain safe and adequate living environment in accordance with California Community Care Licensing standards

Education & Work Experience

Must be 21 years of age or older

Bachelor's degree in social science, psychology or a related field is required, Master's degree preferred

Experience working with young adults (ages required

Familiar with the operations of a transitional living program

Experience with Independent Living Skills issues preferred

Basic computer knowledge and skills

Must be able to work as part of a team

Must be self-aware, flexible, adaptable and teachable

Valid CA Driver's License

Current CDL H6 print-out (must be dated within 30 days of application.)

Pass pre-employment drug testing

Criminal record clearance by being fingerprinted prior to reporting to work and signing a statement regarding criminal convictions

A physical exam and negative TB test results

Ability to adequately observe participants activities, enforce safety regulations and apply appropriate policies and procedures

Current CPR and First Aid certification required and must be from one of the following certifying organizations: American Red Cross, American Heart Association or American Safety & Health Institute
