Responsibilities

• Review medical records, documentation, and provider notes to assign appropriate ICD-9, ICD-10 (International Classification of Diseases), CPT (Current Procedural Terminology), and DRG (Diagnosis-Related Group) codes.
• Ensure accurate coding of diagnoses and procedures in compliance with current coding guidelines and regulations.
• Prepare and submit clean claims for insurance reimbursement using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems.
• Follow up on unpaid or denied claims by investigating discrepancies, resubmitting claims, or initiating medical collections when necessary.
• Maintain detailed and organized medical records, ensuring all documentation aligns with coding standards and legal requirements.
• Collaborate with healthcare providers to clarify documentation or resolve coding issues promptly.
• Stay updated on changes in coding regulations, insurance policies, and healthcare compliance standards.
• Assist in auditing medical records for accuracy and completeness to support billing processes.
• Support the organization's revenue cycle management by identifying billing opportunities and reducing claim rejections.

Skills

• Proficiency in DRG, CPT coding, ICD-9, ICD-10, and ICD coding systems is essential for accurate classification of diagnoses and procedures.
• Strong knowledge of medical terminology, anatomy, physiology, and healthcare documentation standards.
• Experience with medical billing processes including claim preparation, submission, follow-up, and collections.
• Familiarity with EMR/EHR systems used for documenting patient encounters and billing information.
• Ability to interpret complex medical records and translate them into appropriate codes efficiently.
• Excellent attention to detail with a focus on accuracy to minimize claim denials or delays.
• Good communication skills for collaborating with healthcare providers, insurance companies, and patients regarding billing issues.
