Job Title: Community Case Manager

Location: San Francisco Drop In Service Center

1060 Howard St. San Francisco, 94103

Schedule: Monday- Friday 8AM-4PM

Reporting Type: In-Person

Employment Type: Non-Exempt; Full-Time

Salary Range: $29.75/ hour

Summary (of position):

The Community Case Manager (CCM) will support the San Francisco Drop-In Service Center by managing office flow, engaging and following up with clients, completing enrollments and referrals, providing advocacy, data entry and management, resource navigation, crisis de-escalation, socialization, and community building. The CCM works in a fast-paced environment that serves all veterans who are facing a variety of personal or situational challenges.

Responsibilities:

· Engage with veterans to screen, interview, and triage access to services; provide timely and appropriate internal and external referrals.

· Conduct comprehensive case management, including enrollment into the electronic case management platform (ECM), preparation and maintenance of confidential case records, and accurate documentation of services.

· Provide advocacy and assistance with navigating service providers across multiple systems of care, including gathering paperwork such as birth certificates, IDs, and income verification.

· Ensure services are delivered with cultural humility, harm reduction, and trauma-informed practices, while adhering to organizational and funder policies.

· Collaborate with internal departments, clinical teams, and community partners to coordinate client needs, share updates, and address resource-related issues.

· Develop and maintain a network of trusted contacts and referral resources, including internal and external services.

· Conduct follow-up to confirm veterans and/or their families received services and assess further needs.

· Participate in multidisciplinary team meetings, providing input on clients identified as needing case management support.

· Provide crisis intervention and risk management support in collaboration with the clinical team.

· Assist with daily operations of the Drop-In Service Center and perform other tasks as delegated by the Service Center Manager.

· Create a safe, supportive, and welcoming environment that fosters socialization and community building.

Desired Qualifications:

· Knowledge of local social service, public health, and veteran-serving organizations.

· Strong organizational skills; detail-oriented, reliable, and able to manage multiple

priorities.

· Excellent written and oral communication skills.

· Ability to work in a team environment and communicate professionally with internal

and external partners.

· Strong crisis intervention and de-escalation skills with ability to respond

appropriately to conflict and emergencies.

· Ability to set healthy boundaries with at-risk populations.

· Proactive and collaborative problem-solving abilities.

· Comfortable interacting with individuals from diverse backgrounds and experiences.

· Excellent customer service skills and ability to follow through on assigned tasks in a timely manner.

· Flexibility and willingness to perform multiple jobs in a work environment with changing priorities.

· Proficiency with MS Office Suite, web-based applications, and Internet search engines.

· Must be able to perform all essential job functions within reasonable accommodation.

· A veteran of the U.S. military or a military-related family member is preferred.

Experience:

· Minimum of two (2) years' experience providing case management in a social service, behavioral health, or substance use setting.

· Translatable military experience and skills may be alternatively substituted.

Education:

· BA/BS in Social Work or related field strongly preferred (equivalent professional experience accepted)
