This position will cover South Bay, Central Los Angeles, and West Los Angeles areas Ideal candidate will live in Central Los Angeles.

Hospice RN Case Manager Responsibilities:

• Perform RN duties on hospice team.
• Serve as a RN Case Manager for the hospice Interdisciplinary Team
• Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
• Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate
• Coordinates all hospice services provided to patients/families in all settings to ensure quality of care

Ideal Hospice RN Case Manager Candidate Background:

• Registered Nurse with valid CA state license
• Minimum one year of hospice RN experience
• Strong background in medical/surgical, ICU/CCU, or ER is helpful
• Valid driver's license, auto insurance, reliable transportation and willingness to drive to patients place of residence
• Must have excellent interpersonal skills and a passion for hospice
• Understanding of hospice philosophy and needs of the terminally ill
• Self-directed with ability to work with little supervision
• Current Physical and TB test results
