Job Title: Social Worker (LCSW/LMFT)

Location: San Jose, CA
Schedule: Full-Time
Salary: $130,000–$140,000

Job Summary:

We are seeking an experienced Social Worker (LCSW or LMFT) to provide high-quality, culturally competent psychosocial care to pediatric, adolescent, and family medicine patients. This role includes conducting assessments, providing therapy, coordinating care with interdisciplinary teams, and supporting at-risk and high-risk clients with medical, behavioral, and social needs.

Responsibilities:

• Conduct psychosocial assessments for clients and families
• Provide individual counseling/interventions for pediatric and adolescent clients
• Identify at-risk patients and intervene early to prevent further deterioration
• Participate in treatment team meetings and track client progress
• Develop, implement, and evaluate individualized service plans
• Collaborate with families, clinicians, schools, and community resources
• Support mental health resource development and grant-related activities
• Assist with data collection and reporting
• Attend meetings/trainings and comply with agency policies

Qualifications:

• Licensed in California as LCSW or LMFT
• Master's degree in Social Work or related field
• 2+ years experience in healthcare or clinical setting (preferred)
• Experience with pediatric/adolescent populations strongly preferred
• Spanish-speaking preferred
• Strong case management, outreach, and mental health service skills

Pay: $130, $140,000.00 per year
