Job Description

Step into a high-impact Care Coordinator (Case Management) role in beautiful Bakersfield, CA, supporting a busy inpatient acute care setting. This assignment calls for a confident, compassionate nurse who excels in discharge planning, communication, and coordinating care across diverse patient needs—including uninsured and homeless populations.

Roles and Responsibilities

  • Complete initial assessments, discharge planning, and coordination of care
  • Manage a caseload of 15–28 patients per day across inpatient units
  • Arrange post-acute services (nursing homes, home health, DME, prior authorizations, transportation, etc.)
  • Collaborate closely with physicians, bedside RNs, therapists, social workers, and community partners
  • Work with all payer sources including commercial, Medicare, Medi-Cal, uninsured, and high-risk populations
  • Document efficiently in Cerner EMR and maintain accurate case records
  • Provide excellent customer service to patients, families, and interdisciplinary teams
  • Ability to prioritize and adapt to continuously changing needs
  • No call required

Qualifications

  • Minimum 3 years of acute care RN experience
  • Prior care coordination experience required (MUST be from a CommonSpirit/Dignity/CHI facility)
  • Strong discharge planning background
  • Ability to work in a fast-paced, high-volume environment
  • Exceptional customer service and patient-centered mindset

Education

  • ASN or BSN from an accredited nursing school

Required Certifications

  • BLS

Knowledge & Skills

  • Strong interpersonal and written communication
  • Skilled in discharge planning workflows
  • Working knowledge of payers, authorization processes, and community resources
  • Ability to multitask and work independently
  • Proficiency with Cerner EMR
  • Organized, detail-oriented, and able to handle large caseloads
