Position Summary
The Quality Analyst (QA) is part of the Quality team and supports the delivery of high-quality, patient-centered end-of-life services by monitoring clinical performance, ensuring regulatory compliance, and identifying opportunities for improvement. This role focuses on data analysis, interpretation, quality reporting, process evaluations, and collaboration with clinical teams to enhance patient outcomes and caregiver satisfaction.
Essential Functions:
Quality Monitoring & Compliance
- Review clinical documentation to ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, and hospice accreditation standards (e.g., CHAP, HOPE)
- Assist in preparing report for submission of required HOPE data to the Centers for Medicare & Medicaid Services (CMS) within the required timeline.
- Conduct routine audits of patient charts, care plans, IDT (Interdisciplinary Team) notes, symptom impact items (J0501), follow-up visit (SFV) requirements, and visit documentation for accuracy and completeness.
- Monitor compliance with hospice eligibility guidelines, recertification requirements, and timely documentation.
- Assist with preparing for and responding to regulatory surveys or audits.
Data Analysis & Reporting
- Analyze quality indicators (HOPE data) including assessment items related to symptom management, emotional and spiritual support, patient outcomes, symptom management metrics, caregiver satisfaction surveys (e.g., CAHPS Hospice), and operational performance measures.
- Track and report quality and compliance metrics to leadership and clinical teams.
- Identify trends, gaps, and patterns in clinical and operational data, providing actionable recommendations.
Performance Improvement
- Support the development and implementation of Quality Assurance and Performance Improvement (QAPI) initiatives.
- Work with interdisciplinary teams to develop corrective action plans when deficiencies or opportunities for improvement are identified.
- Participate in QAPI meetings, presenting data findings and monitoring progress on quality projects.
- Assist in developing and updating policies, procedures, and clinical guidelines to improve care delivery.
Training & Support
- Provide feedback and coaching to clinical team members on documentation standards, purpose, structure, and best practices with the HOPE tool and other quality requirements.
- Develop and support training initiatives on compliance requirements, workflow improvements, and quality measures.
- Provide EMR training to support clinical outcomes.
- Provide feedback and education to staff on proper assessment and documentation techniques to improve data integrity.
Collaborate with IDT members and clinical leadership to promote a culture of continuous improvement and patient safety through accurate and consistent data collection during all required time points (admission, update visits, and discharge).
Follows company policies to ensure consistent and compliant job performance.
- Performs all other duties as assigned.

Pay Range: starting at $31.63 per hour (based on experience)

Education,CertificatesandExperience:
- Bachelor's degree in nursing, Healthcare Administration, Public Health, or related field. RN is preferred but not required. RNs are required to have licensure in both ND and MN and/or obtain dual licensure upon offer of employment.
- Minimum of 2 years' experience in hospice, home health, palliative care, or other clinical settings.
- Working knowledge of Medicare hospice regulations and industry quality standards.
- Proficiency in EMR/EHR systems commonly used in hospice, with strong analytical and problem-solving skills.
- Experience with quality analytics, chart audits, and performance improvement methodologies.
- Current driver's license and auto insurance
