JOB DESCRIPTION

Program: HOPWA (Housing Opportunities for Persons with AIDS)

Position: HIV Case Manager

JOB SUMMARY

This position assists with the HOPWA program for eligible clients living with HIV/AIDS. Assists clients with accessing a variety of services, including HOPWA vouchers and support services. Provide clients with appropriate referrals. Assist clients in the development of an Individual Service Plan or Housing Stability Plan. Follow-up with clients, as needed. Maintains detailed case notes through AWARDS HMIS. Records, correspondence and telephone log. Case tracking system maintenance and follow-up.

RESPONSIBILITIES

Responsibilities of the HIV Case Manager include, but are not limited to:

  • Assists clients in the development of a Housing Stability Plan and Assessment
  • Explain and interpret federal and local HOPWA regulations and rules to clients as well as interested landlords, realtors and property owners
  • Follow up with clients, as needed
  • Provide clients with appropriate referrals and assist in accessing mainstream resources
  • Through case management, assess and foster independent living skills and facilitate access to mainstream services (examples include, but are not limited to, ADAP and other prescription assistance, health care, Medicaid, Food Stamps, mental health, Social Security, HOPWA) through thorough intake
  • Meet or contact client monthly to review housing plan
  • Explains program rules and expectations, and client's obligations and rights under program, including grievance and discharge policy
  • Assists clients in identifying suitable housing and negotiating rental agreements, including review of tenant/landlord leases for compliance with federal, state and local laws
  • Update the clients Authorization for Use & Disclosure of Information to be included
  • Available to meet with landlords to review program paperwork and secure a written agreement to participate in program
  • Monitors inspection of properties prior to occupancy, during annual recertification, to meet housing quality standards
  • Maintains a confidential client record and documents all work related to the client's status.
  • Offer linkage to care for those who have fallen out of care
  • Perform other relevant duties as assigned

KNOWLEDGE, ABILITIES AND SKILLS- REQUIRED AT TIME OF HIRE

  • Excellent customer service skills
  • Highly proficient in MS Word, Outlook, Calendar scheduling, EXCEL, Publisher and PowerPoint and ability to learn new software systems
  • Able to manage multiple projects with committed deadlines and consistently demonstrate high attention to detail
  • Highly organized, comfortable following processes within a dynamic team environment
  • Effective communication with clients, all levels of management and partnering agencies
  • Possess solid problem solving and multi-tasking skills
  • Ability to serve the public and fellow employees with honesty and integrity within the letter and spirit of ASC Employee Policies and Procedures
  • Ability to establish and maintain effective working relationships with the general public, co-workers, and members of diverse cultural backgrounds, regardless of race, color, religion, age, gender, ethnicity, disability, sexual orientation, marital status or political affiliation.
  • Must have a valid driver license and clean driving record and personal auto insurance.
  • Must show proof of COVID-19 vaccination.

REQUIRED TRAINING AND EXPERIENCE

  • Two (2) years related office work experience preferred
  • Bachelor's Degree in Social Work or Related Field, Master's Degree preferred
  • Three (3) years related work experience preferred

SALARY RANGE

$30,000 to $35,000, depending upon qualifications and experience

Job Type: Full-time

Pay: $30, $35,000.00 per year
