Job Summary
We are seeking a highly skilled and compassionate Family Nurse Practitioner to provide comprehensive primary care across diverse patient populations. The ideal candidate will possess extensive clinical expertise, including experience with acute and chronic conditions, pediatric and geriatric care, and a strong background in medical documentation and EMR/EHR systems such as Epic, Cerner, and eClinicalWorks. This role involves delivering patient-centered care in outpatient, inpatient, telehealth, and assisted living settings, with a focus on improving health outcomes through evidence-based practices and effective case management. Candidates with specialized skills in areas like dermatology, pain management, infectious disease care, or hospital medicine are encouraged to apply.
Duties
- Conduct thorough patient assessments including vital signs, physical examinations, and medical history reviews.
- Manage acute and chronic illnesses across various specialties such as internal medicine, geriatrics, pediatrics, emergency medicine, and behavioral health.
- Perform procedures including suturing, IV insertion, catheterization, venipuncture, phlebotomy, sterile processing techniques, and basic life support.
- Administer injections and medications accurately while adhering to HIPAA regulations and infection control protocols.
- Utilize EMR/EHR systems like eClinicalWorks for documentation of patient encounters and medical records management.
- Provide specialized treatments such as botulinum toxin therapy, dermal fillers, esthetic laser treatments, spinal taps, gastric lavage, or dialysis as applicable.
- Engage in diagnostic evaluation using laboratory testing or medical imaging; interpret ICD-9/ICD-10 coding for billing purposes.
- Coordinate discharge planning, post-acute care transitions, and utilization review to optimize patient outcomes.
- Offer health coaching on nutrition, disease prevention (including family planning), medication adherence, and lifestyle modifications.
- Collaborate with multidisciplinary teams across hospital medicine units including ICU/Level I & II trauma centers or neonatal/pediatric intensive care units (PICU).
- Participate in telehealth consultations to extend care access; provide triage for urgent care needs or emergency room referrals.
- Maintain compliance with all regulatory standards including NCQA standards and DME protocols while ensuring infection control measures are strictly followed.
Experience
- Proven experience working in diverse clinical settings such as hospitals (including Level I & II trauma centers), outpatient clinics, nursing homes, hospice & palliative medicine units, or assisted living facilities.
- Demonstrated expertise in managing complex cases involving acute pain management, infectious diseases (including COVID-19), with STI and HIV geriatrics/dementia/dementia care/memory care/Alzheimer's care.
- Hands-on experience with advanced procedures such as cardiac catheterization or ventilator management is preferred.
- Familiarity with medical coding (ICD-9/ICD-10/CPT) for billing accuracy; proficiency in EMR/EHR systems like Epic or Cerner is essential.
- Background in pediatric (including neonatal ventilator use), neonatal intensive care (NICU), or post-operative discharge planning is advantageous.
- Experience working with people with disabilities or developmental disabilities is a plus; knowledge of Workers' Compensation law is beneficial.
- Strong understanding of physiology knowledge and diagnostic evaluation techniques; ability to perform physical examinations effectively. Pay: $93, $124,841.04 per year
