Physical Therapist
Pawtucket Falls is seeking a licensed and motivated PRN Physical Therapist to join our long-term care/nursing home team. The PT will provide rehabilitative services to residents, helping them maintain or improve mobility, strength, balance, and overall function. The role includes assessments, individualized treatment planning, hands-on therapy, documentation, and team collaboration — all aimed at enhancing residents' quality of life and independence.
Qualifications:
- Doctorate (DPT) or Master's/Bachelor's degree in Physical Therapy from an accredited institution — licensed to practice in Rhode Island.
- Previous experience working in geriatrics, skilled nursing, long-term care, or rehabilitation settings preferred (though new grads may be considered depending on facility needs).
- Strong knowledge of therapeutic exercise, manual therapy, mobility training, and assistive devices.
- Excellent interpersonal and communication skills — ability to work with elderly residents, families, nursing staff, and other therapists.
- Ability to document care accurately and meet regulatory/payer documentation requirements.
- Compassion, patience, and a commitment to improving resident quality of life and independence.
Responsibilities:
- Conduct comprehensive physical assessments of residents' mobility, strength, gait, balance, and functional abilities.
- Develop individualized care/treatment plans and set realistic short- and long-term goals for rehabilitation.
- Provide physical therapy interventions: manual therapy, therapeutic exercises, gait and balance training, mobility assistance, use of assistive devices (walkers, braces, wheelchairs), and other modalities as appropriate.
- Supervise and collaborate with Physical Therapy Assistants (PTAs) when applicable.
- Educate residents, family members, and nursing staff on safe mobility, transfers, exercise programs, fall prevention, and the use of assistive devices.
- Document treatments, progress notes, evaluations, and discharge summaries in compliance with regulatory, facility, and payer requirements.
- Work as part of an interdisciplinary care team (nursing, occupational therapy, nursing aids, social services) to ensure coordinated resident care.
- Maintain therapy equipment, treatment areas, and ensure a safe, clean environment.
