We are hiring a new Case Manager to join our our team. In this role, you will support clients in achieving greater independence and stability while living in the community. Working collaboratively within a multidisciplinary team, you will provide services in psychiatric rehabilitation, medication and symptom management, relapse prevention, harm reduction, independent living skills, wellness coaching, housing support, budgeting, resource linkage, and community integration. This is a full-time position, working Tuesday - Saturday, 11 am to 7 pm. Bilingual in Spanish and English is highly desired. Pay will be commensurate with degree and education.
RESPONSIBILITIES
Administrative Responsibilities
- Meet agency billing and documentation standards within required timeframes.
- Maintain accurate, timely, and complete client records and data tracking related to performance outcomes.
- Communicate promptly and professionally with internal teams and external providers via phone, email, and in-person contact.
Clinical Responsibilities
- Develop, implement, and monitor individualized service plans that align with program philosophy and client goals.
- Support clients with daily living skills such as meal planning, cooking, shopping, and household tasks, and teaching skills where needed.
- Observe and report changes in client behavior, mood, and functioning. Maintain awareness of cultural and physical/mental health needs.
- Promote adherence to safety protocols and model appropriate safety practices.
- Provide individual and group counseling, case management, and skill development that fosters recovery, responsibility, and positive social engagement.
- Assist clients with system navigation (e.g., medical, psychiatric, legal, or detention facilities) and linkages to community support like peer-based groups (e.g., AA, NA, Wellness Centers).
- Provide crisis intervention and management in coordination with supervisory or licensed staff.
- Facilitate groups, classes, or community-based activities as directed.
- Monitor and support medication adherence, including prompts, refills, proper storage, and reporting per program guidelines.
- Participate in program planning and represent the team on agency committees as assigned.
- Provide transportation using personal or agency vehicles and assist with the use of public transportation systems. Ensure clients attend to all medical and related appointments.
Maintenance Responsibilities
- Perform minor repairs at agency sites or residences; report major repair needs to the supervisor promptly.
Program Quality and Compliance
- Ensure all services meet agency quality standards and support measurable client progress and satisfaction.
- Maintain compliance with all regulatory, legal, and contractual requirements (e.g., CARF, Medi-Cal, HUD), including licensing and program certifications.
Case Manager I (Training Track Eligible ):
Buckelew recognizes that valuable skills and commitment to the mental health field can come from diverse educational and experiential backgrounds. Staff who do not yet meet Medi-Cal MHRS criteria may be hired under Buckelew's Training Plan for Other Providers, with a structured training and supervision program as outlined below.
- Highschool Degree with 2 years experience: Eligible with six-year training plan
- Associate degree and no experience: Eligible with six-year training plan
- Bachelor's degree and no experience: Eligible with four-year training plan
- Master's degree and no experience: Eligible with two-year training plan
- Individuals with partial experience in related fields (for example, homeless services, substance use treatment, behavioral health, or case management) may have their required training period reduced proportionally based on prior relevant experience.
- Training includes weekly case conferences, monthly supervision with APD or PD, all Buckelew and County-assigned trainings, and completion of Relias modules in areas such as trauma-informed care, crisis intervention, motivational interviewing, and cultural competence.
Case Manager I (Fully Qualified MHRS ):
Must meet Medi-Cal criteria for a Mental Health Rehabilitation Specialist:
- Highschool Diploma with 8 years or more of experience in mental health experience.
- Associate degree with six or more years of experience in mental health, physical restoration, social adjustment, or vocational adjustment; or
- Bachelor's degree with four or more years of experience in those fields; or
- Master's degree with two or more years of related experience.
Case Manager II:
- Master's degree or higher is required.
- Experience with individuals with mental health and/or substance use disorders is preferred.
- Licensure (MFT, LCSW, etc.) or progress toward licensure is preferred.
- Some program placements may require licensure based on contractual obligations.
Professional Skills
- Fluent in English; bilingual in English/Spanish highly preferred.
- Ability to work independently with strong organization, flexibility, empathy, and sound judgment.
- Demonstrated professionalism and the ability to maintain healthy boundaries.
- Strong interpersonal skills with the ability to relate to clients, colleagues, and external partners.
- Intermediate proficiency in Microsoft Office (Excel, Word, Outlook).
- Experience with Electronic Health Record (EHR) systems strongly preferred.
- Must be a reliable, adaptable, and collaborative team member.
- Valid California driver's license, insurable driving record, and access to an insured vehicle required.
- Must pass Live Scan background check (as required by some programs).
PHYSICAL REQUIREMENTS
- Ability to intermittently perform physical tasks such as bending, stretching, squatting, reaching, lifting (up to 30 lbs.), and driving.
- Extended periods of sitting, typing, writing, standing, and driving may be required.
