Support individuals with intellectual disabilities in a small home settings in accordance with Federal, State, and Local standards, as well as our established policies and procedures, focusing on recommendations contained in the Individuals' Personal Plan.
MAJOR DUTIES AND RESPONSIBILITIES:-
-Attend DSP classes as assigned by the CILA Coordinator/QIDP as needed
Conduct daily observations and documentation
Promote team work by assisting co-workers in the care of residents regardless of staff assignments
Ensure that assigned documentation is current and appropriately completed
Comply with written behavior programs and document behavioral incidents in individual client files
Ensure that clients are well groomed and appropriately attired for the weather and that clothes fit properly and are presentable
Assist in dining area during meal time to ensure client needs are being met through monitoring and structured programs
Ensure normalized training and interaction during meal periods with clients
Assist clients in self-medication programs
Complete home and client laundry as directed
Maintain a clean and safe environment through performance of daily cleaning
Assist clients on doctor appointments, dentist appointments, etc
Monitor individual client supplies for cleanliness and appropriateness
Maintain CONFIDENTIALITY of all pertinent client care information to assure patient rights are protected
Maintain CONFIDENTIALITY of all pertinent home records
Perform other specific duties if requested regardless of routine schedule within training and capabilities
Perform other special duties as designated by CILA Coordinator/QIDP or Executive Director
EDUCATIONAL REQUIREMENTS:
Must have a High School Diploma or Equivalent. Must have completed or be willing to complete DSP training
ESSENTIAL FUNCTIONS:
Must be able to accept change of job schedule resulting in overtime without prior notice due to the health care provided by out facilities to persons with developmental disabilities
Must attend all in-service training and staff meetings
Must have knowledge of and adhere to client rights and regulations regarding privacy issues
Follow facility vehicle policies regarding cleanliness, no smoking, recording of mileage, etc
Assist in training new personnel regarding the facility, its policies and procedures,and to job position and duties
Required to assist non-ambulatory clients in transferring for short duration, sit, stand, bend, lift, and move intermittently during work hours
Required to assist partially ambulatory clients in walking using a gait belt
Required to assist clients with their self-care such as bathing, toileting, eating, etc
Must be screened by finger-printing to assure no listing of convictions of previous crimes or abuse
Must be at least 18 years of age, of temperate habits and good moral character, honest, reliable, and trustworthy
Required to have a valid drivers license to operate facility vehicle and to be considered insurable based on criteria by the agency insurance carrier
Must be willing to undergo criminal and/or abuse record checks to determine history of conviction for violent crimes or abuse of disabled individuals, including elderly or children
Must assist with completion of assessments, consents and all other documentation as designated
