Physical Therapist (PT) - Hope Home Care - Montgomery & Miami County

Compensation: Guaranteed Base, Uncapped Potential, $90,000 - $110,000+ commensurate with experience, processed bi-weekly.

Shift Type: Full-Time, Monday - Friday, 8/9 AM to 4/5 PM (flexible).

Summary:

The Physical Therapist ("PT") is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment. A PT provides therapy services, including the creation and administration of care plans, as well as oversees physical therapy assistants. A PT must work collaboratively across the internal interdisciplinary care team, physicians, and other stakeholders in our patient's care.

Responsibilities:

  • Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
  • Provide and complete all proper documentation.
  • Administer physical therapy treatments as ordered by a physician. Direct and oversee treatment given by physical therapy assistants.
  • Evaluate the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications.
  • Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and/or providing equipment; recommending outpatient follow-up programs).
  • Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.

Qualifications:

  • A minimum of 2 years of physical therapy experience is preferred with a preference for 2+ years in an acute or a post-acute care setting.
  • Active state licensure in physical therapy is required.
  • Proof of current CPR certification and valid driver's license.
  • Proof of current auto insurance coverage.
