Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in child and family counseling, mental health counseling, and a strong understanding of case management. This role involves providing support to individuals and families facing various challenges, conducting assessments, and developing treatment plans to enhance their well-being. The Social Worker will work collaboratively with healthcare professionals to ensure comprehensive patient care.

Responsibilities

  • Conduct patient assessments to identify needs and develop appropriate care plans.
  • Provide individual and group counseling sessions to clients dealing with mental health issues, addiction, or family dynamics.
  • Engage in crisis intervention and management strategies to support clients in distress.
  • Maintain accurate medical documentation and adhere to HIPAA regulations.
  • Collaborate with multidisciplinary teams in inpatient, outpatient, and community settings.
  • Facilitate group therapy sessions focusing on behavioral therapy techniques.
  • Perform intake evaluations for new clients, ensuring thorough data collection.
  • Offer support for patients in hospice care, assisted living facilities, or nursing homes.
  • Utilize EMR systems for effective record keeping and patient management.
  • Stay informed on clinical research and best practices in social work.

Requirements

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Valid state licensure as a Social Worker.
  • Experience with behavioral health, including PTSD care, dementia care, or working with people with developmental disabilities.
  • Proficiency in medical terminology and familiarity with ICD-9/ICD-10 coding.
  • Ability to work effectively with children, families, seniors, and individuals facing diverse challenges.
  • Knowledge of case management principles and crisis intervention techniques.
  • Excellent communication skills and the ability to build rapport with clients from various backgrounds.
