Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in child and family counseling, mental health counseling, and a strong understanding of case management. This role involves providing support to individuals and families facing various challenges, conducting assessments, and developing treatment plans to enhance their well-being. The Social Worker will work collaboratively with healthcare professionals to ensure comprehensive patient care.
Responsibilities
- Conduct patient assessments to identify needs and develop appropriate care plans.
- Provide individual and group counseling sessions to clients dealing with mental health issues, addiction, or family dynamics.
- Engage in crisis intervention and management strategies to support clients in distress.
- Maintain accurate medical documentation and adhere to HIPAA regulations.
- Collaborate with multidisciplinary teams in inpatient, outpatient, and community settings.
- Facilitate group therapy sessions focusing on behavioral therapy techniques.
- Perform intake evaluations for new clients, ensuring thorough data collection.
- Offer support for patients in hospice care, assisted living facilities, or nursing homes.
- Utilize EMR systems for effective record keeping and patient management.
- Stay informed on clinical research and best practices in social work.
Requirements
- Master's degree in Social Work (MSW) from an accredited institution.
- Valid state licensure as a Social Worker.
- Experience with behavioral health, including PTSD care, dementia care, or working with people with developmental disabilities.
- Proficiency in medical terminology and familiarity with ICD-9/ICD-10 coding.
- Ability to work effectively with children, families, seniors, and individuals facing diverse challenges.
- Knowledge of case management principles and crisis intervention techniques.
- Excellent communication skills and the ability to build rapport with clients from various backgrounds.
