Summary

The Cornerstone Case Management Clinical Coordinator will lead the creation and implementation of the case-management model at the Cornerstone Program. Utilizing a strength-based, youth-centered, and trauma-informed lens, the Social Worker shall provide direct services ranging from prevention of high-risk activities to crisis intervention for Participants, their families, and the immediate community. Specifically, the Social Worker shall perform functions including but not limited to the tasks listed below:

Key Responsibilities:


• Initiate contact and establish trust amongst identified high risk Participants currently in the Cornerstone program; as well as implement outreach strategies to engage new participants, within the primary target age of 18-21 year olds.


• Conduct intakes and holistically assess each Participant's strengths and needs and develop an Individual Service Plan ("ISP"). The ISP shall include, at a minimum, goals for education, employment, health, and life skills for the Participant.


• Identify Participants who have experienced or are experiencing trauma and establish weekly Healing Circles for them and high-risk groups.


• Supervise and support the Navigator to implement the ISPs.


• Work with Participants to foster positive relationships with peers and adults.


• Support Participants' family needs and provide/refer those in need to additional resources and services and serve as a supportive resource to other on-site staff at the Cornerstone community center.


• Complete documentation and progress notes in a timely manner.


• Performs other related duties as assigned.

Supervisory Responsibilities:

Manages the overall direction, coordination, and evaluation of the Program. Carries out supervisory responsibilities in accordance with the agency policies and applicable laws. Responsibilities include interviewing, hiring, planning assigning, directing work, rewarding, disciplining employees, performance reviews, and addressing complaints and resolving problems.

Minimum Qualifications:


• LMSW/LCSW certification.


• Proficiency in Microsoft Office suite.


• Ability to plan and carry out assignments independently.


• Ability to prioritize, adhere to timelines and multi-task.

Qualifications Preferred:


• Comfort and knowledge of trauma work, evidence-based treatments and youth development.


• Excellent communication skills, especially writing.


• Adept at developing strong working relationships across diverse groups.
