Job Summary:

Burlington Area School District is seeking a nurse care manager. This position is highly flexible and rewarding. The nurse will work with a small population of patients and have an impact on their health and well-being. We are a small locally owned business who values their employees and understands the importance of work life balance. No Holidays or weekends are required.

The role of this nurse is to facilitate and ensure continuity in patient care and serve as a conduit of care between the services of the BASD Health Clinic, the District's third-party administrator, and the District's health network. The Advocate will strategically engage employees and their dependents before, during, and after visits to the clinic to ensure we are providing personalized, informative, and compassionate care. This role ensures that all patients receive high-quality, compassionate, and coordinated healthcare services. The advocate will assist patients in navigating the healthcare system, addressing concerns, and ensuring they have the necessary resources to achieve optimal health outcomes.

Key Responsibilities:

  • Assist patients in understanding their diagnoses, treatment options, and healthcare plans.
  • Advocate for patients' needs by ensuring their voices are heard and respected within the healthcare system.
  • Provide guidance on insurance benefits, medical billing, and available community resources.
  • Support patients in scheduling and coordinating appointments, follow-ups, and referrals.
  • Educate patients on preventive care, wellness programs, and available clinic services.
  • Collaborate with healthcare providers, social workers, and administrative staff to optimize patient care.
  • Address patient concerns and complaints, working towards effective resolutions.
  • Maintain patient confidentiality in compliance with HIPAA regulations.
  • Document patient interactions and advocacy efforts accurately and promptly.

Qualifications:

  • Bachelor's degree in healthcare administration, social work, nursing, or a related field (preferred).
  • Current BLS certification through the American Heart Association valid for at least 90 days after start date.
  • Experience in patient advocacy, healthcare navigation, or a similar role.
  • Strong knowledge of healthcare systems, insurance processes, and patient rights.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while collaborating effectively within a healthcare team.
  • Proficiency in medical terminology and electronic health records (EHR) is a plus.
  • Compassionate, patient-centered approach to healthcare.
  • Knowledge of HIPAA confidentiality requirements

Preferred:

  • Knowledge of third-party payer requirements

Work Schedule & Compensation:

  • Flexible scheduling within clinic hours, totaling 30-40 hours per week.
  • Hourly wage ranging from $30 - $45, based on experience and qualifications.
