Hospice & Community Care of Rock Hill, SC is actively hiring an RN Case Manager to work Full Time

Shift Details: Monday - Friday (8:00am - 4:30pm)

As a nonprofit deeply rooted in the Rock Hill, SC community, we go beyond traditional hospice services; offering charity care, grief support, a free medical equipment closet, and dedicated programs for our veterans.

We are currently seeking a Full-Time RN Case Manager to bring comfort, dignity, and compassion to patients and families during life's most critical moments. If you're looking to make a meaningful impact every day, not just in your work, but in your community, we'd love for you to join our mission.

What You'll Do:

  • Assess hospice patients, coordinate and implement care plans, and evaluate interventions for hospice patients.
  • Lead hospice care team members in coordinating services for a caseload of patients in accordance with the physician.
  • Provide outstanding nursing care, including teaching and supporting the patient as well as the caregiver. Care is provided through in-person visits and telehealth support.
  • Present services to potential patients, obtain initial physician orders, and complete all necessary paperwork related to nursing admissions and other visits as necessary.

What You'll Need:

  • Registered Nursing license in South Carolina, or be eligible for licensure in South Carolina.
  • Valid CPR certification
  • Experience in healthcare, home care, or hospice setting

Application Question(s):

  • Do you have a valid RN License?
  • Do you have a valid Driver's License and reliable vehicle to make patient visits?
  • This position requires travel to hospice patients homes or facilities to administer your visits. Our patient service area is up to 60 minutes from our location in Rock Hill, SC. Are you comfortable with this for the role?
  • This position includes a rotating back up on-call requirement, with a maximum of 7 shifts per month. Are you available and willing to take on-call shifts?
