Program Specialist - 60,000 Annually

Spectrum Community Services provides services to residents with intellectual and developmental disabilities (IDD) in a residential setting. The Program Specialist is responsible for overseeing the development, implementation, and evaluation of individualized service plans (ISPs) for residents. This role ensures compliance with state regulations, supports staff training, and advocates for person-centered care to enhance residents' quality of life.

SCHEDULE: Monday - Friday: 8 AM - 5 PM but on call 24/7

PAYRATE: $60,000 per year

Location: Wyomising, PA 19610

Key Responsibilities:

• Develop, monitor, and revise Individualized Service Plans (ISPs) to ensure person-centered care.
• Monitor health, behavioral, and social progress while coordinating necessary support services.
• Ensure adherence to state, federal, and agency regulations for IDD services.
• Maintain accurate and timely documentation of resident progress, incidents, and ISP updates.
• Complete monthly and quarterly reports on each individual assigned to the caseload.
• Ensure AIS/SH/HCH is applied for as needed and that renewal applications are reviewed by the team and completed one month prior to the expiration of the service
• Ensure all residents receive required and necessary medical/dental/psychiatric/all other care/appointments in a timely manner and ensure follow-up occurs on all recommendations by the individual's health care provider.
• Assist with transportation coordination for medical appointments, employment programs, and community outings.
• Acts as HCSIS Point person and enters and investigates incident reports
• Ensures that the programs, vehicles, and equipment are safe, functional, and secure for the individual's physical well-being

Qualifications:

• Bachelor's degree in Social Work, Psychology, Special Education, Human Services, or a related field (required).
• Minimum 2 years of experience working with individuals with IDD in a residential or community-based setting.
• Strong knowledge of IDD best practices, person-centered planning, and positive behavior support strategies.
• Familiarity with state regulations, Medicaid waivers, and HCBS settings for IDD services.
• Excellent organizational, problem-solving, and interpersonal skills.
• Ability to train, lead, and support direct care staff in ISP implementation.
