Summary

Atlantis Community Inc. has been advocating for people with disabilities for 49 years. Atlantis serves a seven county catchment area (Adams, Arapahoe, Clear Creek, Denver, Douglas, Elbert, and Jefferson Counties). As a Center for Independent Living, we support the rights of people with disabilities to direct their own lives by providing advocacy, life skills training, transition services (youth, from nursing homes, and diversions from institutions), information and referral, and peer support. The mission of Atlantis is to advocate for all people with disabilities to be a meaningful part of an integrated community of their choice. 

The Transition Coordinator (TC) supports people with disabilities in achieving independent living by coordinating transitions from nursing facilities to the community, diverting unnecessary institutional placements. This role is responsible for developing person-centered transition plans, connecting consumers with housing, health care, employment, community resources, and advocating across agencies to remove barriers. The TC ensures compliance with program requirements, maintains accurate documentation, and empowers individuals to make informed choices about their lives while promoting disability rights and full community inclusion. This position reports to the Director of Transitions.

To be successful in this role, a Transition Coordinator must be passionate about promoting independent living and disability rights, skilled in building trust with diverse individuals, and able to navigate complex systems such as Medicaid, housing, and community services. The TC should demonstrate strong organizational and advocacy skills, practice person-centered planning, and maintain professionalism and sensitivity when working with people with disabilities, families, and community partners. Success in this role also requires adaptability, attention to detail in documentation, and the ability to coordinate resources that support consumers in achieving their goals for independence.

Essential Duties and Responsibilities

Consumer Services

  • Coordinate nursing home transitions maintaining compliance with Colorado's Medicaid program Transition Coordination Management- Transition Services (TCM-TC).
  • Support ACI's nursing home diversion and re-entry grant program by meeting with community partners, referred consumers, and providing person-centered transition planning.
  • Lead, organize, and facilitate all transition teams, meetings, services, and supports pre- and post-transition.
  • Complete and maintain all necessary documentation for transition services (with team and consumer input), including assessments, plans, applications, referral forms, etc.
  • Provide individual advocacy to ensure consumer rights and needs are met.
  • Assist consumers in acquiring necessities for success in the community, including housing, transportation, employment, social/recreation opportunities, volunteer opportunities, independent living skills training, advocacy, and peer support.
  • Work with clients on an as-needed basis after transition services are complete.
  • Serve as a mediator to help resolve conflicts related to transitions and advocate for the best interests of consumers.
  • Accompany consumers to community-based appointments as needed.
  • Develop a strong peer support network to connect consumers with individuals who have successfully transitioned.
  • Communicate professionally and effectively with consumers, families, and agency staff.

Community Involvement

  • Coordinate and collaborate with agencies including Single Entry Points, Community Centered Boards, Home Health Care agencies, and Regional Accountable Entities to secure post-transition service
  • Develop professional relationships with nursing homes, assisted living, state hospitals, community mental health centers, home health agencies, and local, state, and federal government agencies (such as HUD and CMS).
  • Be knowledgeable of and actively involved in state and federal legislative bills that pertain to transitions.

Operations

  • Maintain thorough, complete, timely, and accurately organized documentation in both the state Medicaid system (CCM) and ACI's consumer documentation system (CILsuite).
  • Submit accurate and timely claims for reimbursement of services provided.
  • Assist staff in gathering and collecting data about the transition program for grants, board meetings, and fundraising projects/events.
  • Maintain compliance with HIPAA and strict confidentiality of all information concerning consumers, staff, volunteers, and board members, including maintenance of physical files.
  • Meet deadlines for completion of monthly paperwork and ongoing reports to both state and federal agencies.
  • Perform other duties as assigned.

Essential Qualifications

  • Must possess and maintain a valid Colorado Driver's License (CO DL).
  • Must have reliable transportation available for work-related travel.
  • Must carry and maintain current automobile insurance as required by Colorado law.

Education and Experience

  • A bachelor's degree;
  • Five (5) years of relevant experience in the field of LTSS, which includes Developmental Disabilities;
  • or some combination of education and relevant experience appropriate to the requirements of the position

.

Relevant experience is defined as:

a. Experience in one of the following areas: long-term care services and supports, gerontology, physical rehabilitation, disability services, children with special health care needs, behavioral science, special education, public health or nonprofit administration, or health/medical services, including working directly with persons with physical, intellectual or developmental disabilities, mental illness, or other vulnerable populations as appropriate to the position being filled; and, 

b. Completed coursework and/or experience related to the type of administrative duties performed by case managers may qualify for up to two (2) years of required relevant experience. 

Preferred Qualifications

  • Lived experience with a disability preferred
  • Experience with documentation standards for Medicaid reimbursement
  • Experience promoting the social model of disability navigating healthcare and community barriers
  • Experience in rehabilitative roles promoting consumer's independence and autonomy, examples include occupational therapy assistants (OTAs) and physical therapy assistants (PTAs)
  • Proficiency in American Sign Language (ASL) or Spanish preferred

Physical Demands

  • Must be able to sit for 30–60 minutes at a time at a desk.
  • Must be able to practice self-care techniques at work to avoid occupational burnout.
  • Position may occasionally require assisting consumers with light physical tasks related to transitions, such as carrying groceries, moving personal items, or assembling small furniture.
  • Heavy lifting is not a regular requirement; assistance from external vendors (e.g., movers, TaskRabbit) may be used for larger items.

Compensation/Benefits

  • Hourly wage- $33.05
  • ICHRA Medical Plans with ACI paying 100% of base premium for the employee with buy-up options available, and a Health Reimbursement of up to $500.00/Month to assist with dependent medical coverage

Important Notes

  • This position is Full Time, fully benefited, 32 hours a week
  • Office hours are Monday through Thursday, 8:30am-5:00pm. Schedules will be set between those times. The office closes from 12:00pm - 12:30pm for lunch daily.
