Summary:

The Case Coordination Manager is accountable for the ongoing interactions of the Case Coordinators and Social Workers with patients, physicians, and other health providers to meet designated clinical, operational, and financial outcomes for aggregate patient populations. This role will promote staff achievement in assisting physicians, patients, families, and payors in providing care for patient populations that is appropriate, effective, and cost efficient. This role will hold staff accountable for active attendance and participation in all departmental and interdepartmental meetings relative to Case Coordination and Social Work functions. This role will provide ongoing education to staff and others on effective allocation of the hospital's resources while encouraging provision of high quality patient care. Is cognizant of regulations and policies pertaining to patient rights and Discharge Planning Conditions of Participation. This role functions with advanced skills in the role of clinician, educator, researcher, manager and advocate for the Case Coordination team. This role is responsible for providing clinical leadership on complex cases with the Case Coordination staff. This role will act as an extension of the Director of Case Coordination for assigned hospital(s). This role will either manage or oversee the completion of annual evaluations, terminations, discipline, action plans, payroll, daily schedules, on call schedules, and employee engagement plans.

Education:
*BSN required

• Masters Degree in nursing, business, or related field preferred

Experience:
*2 years of management/leadership experience

• 5 years of Case Management experience with minimum of 2 years in a hospital setting

Credentials/Requirements:
*Registered Nurse in good standing with the state of Arkansas

• CCM or ACM preferred

This job will be authorized 80.00 hours bi-weekly.
