Licensed Practical Nurse LPN

Agawam RehabNursing Campus is seeking a caring Licensed Practical Nurse LPN to join our team full-time, part-time or PRN at our skilled nursing facility.

Through strong personal relationships and thoughtful decision-making, each Licensed Practical Nurse LPN is entrusted to help support a meaningful life for, and with, the elders who are in our care.

Responsibilities:

• Provide direct patient care.
• Conduct patient assessments and maintain accurate electronic health records (EHR).
• Administer medications and IVs as prescribed.
• Collaborate with healthcare teams to develop and implement care plans.
• Ensure compliance with HIPAA regulations and maintain patient confidentiality.
