Job Title: Licensed Counselor Social Worker Therapist SUDS

Seeking: Licensed Social Worker Independent Clinical, Licensed Mental Health Professional, or Licensed Mental Health Counselor or 3 years SUD-P

Hiring 1 full time (Social Worker/Therapist/Counselor) w/ 3 years of SUD-P certification

Hiring 2 hybrid roles

Pay: $75, $90,000 per year

At Heritage, Behavioral Support Specialist (BSS) is the official title for our licensed counselors, social workers, and therapists who provide both behavioral planning in the community and traditional counseling services in the office, tele-health and in the field. In this hybrid role, you'll serve clients through traditional counseling sessions as well as behavioral support planning in a variety of care settings.

Our case loads are small and are built to match the strengths and skills of each clinician. Those with stronger 1:1 Cognitive Counseling skills are given case loads that support those clients. Clinicians that enjoy more behavioral support/planning would have a case load that is serves more clients with writing Behavioral Support plans.

Community-Based Work (BSS)

  • Commute regularly throughout Clark and Cowlitz Counties to meet clients where they live and receive care (homes, apartments, independent housing, AFHs, ALFs, SNFs, and hospitals).
  • Assess behavioral needs, create intervention plans, and collaborate with care teams to support improved quality of life.

Counseling Services

  • Provide individual counseling sessions (in person or via telehealth).
  • Maintain a caseload of counseling clients, supporting their emotional and mental health needs.
  • Use evidence-based practices to support client goals and resilience.

Duties

Behavioral Support Responsibilities:

  • Conduct comprehensive behavioral assessments in client environments.
  • Develop and implement individualized behavior intervention plans.
  • Monitor and adjust plans based on ongoing data and observations.
  • Train caregivers and collaborate with professionals on behavior strategies.

Counseling Responsibilities:

  • Provide therapeutic counseling (in person or via telehealth).
  • Maintain documentation per HIPAA and ethical standards.

General Responsibilities:

  • Maintain detailed documentation and timely reporting.
  • Work with interdisciplinary teams (case managers, therapists, families).
  • Attend staff meetings, trainings, and case reviews.

Skills and Qualifications

  • Licensure: Must hold current certification in Washington State as LSWIC, LMHP, or LMHC, 1 position needs to also have (SUD-P)
  • Technical Proficiency: Microsoft 360, Excel, Word, Outlook, SharePoint.

Work Environment & Flexibility

This hybrid position includes a flexible schedule after onboarding, combining work in the community, office, home, and telehealth.

First 90 Days: Structured schedule, Monday–Friday, 8:15 AM–5:15 PM for training and support.
After 90 Days: Building schedule, balancing home, field, and office work.
