The Case Manager for Enhanced Case Management Program will be embedded within the Homeless Services Division teams. This position will work with homeless individuals with medical, mental health, or substance abuse disorders.  The Enhanced Case Manager will be the single point of contact for our medically vulnerable participants with complex needs who require personalized support to coordinate medical, behavioral, and social services.

What you bring to the table:

  • Bachelors Degree in Social Work, Psychology, or a related field.
  • OR, Associates Degree in Social Services, Case Management, or a related field, plus 2 years related work experience. 
  • OR, 4 years related work experience.
  • As a Enhanced Case Manager you will:

  • Coordinate Admissions: Interface with healthcare facilities to address admission requests and ensure appropriate client placement through pre-admission meetings.
  • Assess & Plan: Evaluate client eligibility for ECM services; complete program assessments and develop care plans with short- and long-term goals.
  • Specialized Knowledge: Maintain understanding of chronic homelessness and related medical/behavioral issues (PTSD, TBI, substance use, mental health).
  • Collaborate: Work with County Behavioral Health Therapists and other providers for therapy referrals and coordinated care.
  • Appointment Management: Ensure timely follow-through for medical, mental health, and substance use appointments; provide transportation as needed.
  • Advocate for Clients: Minimize barriers to accessing services and support client needs.
  • Professional Standards: Maintain confidentiality, adhere to program policies, and monitor compliance with goals and expectations.
  • Communication: Engage daily with clients and shelter staff; communicate weekly with RN and regularly with community partners and funders.
  • Monitor & Adjust: Track progress on care plans, re-evaluate needs, and modify goals or strategies as appropriate.
  • Documentation: Complete logs, timesheets, reports, and client documents; maintain weekly statistical data and submit to hospitals and CenCal.
