Job Summary
The Case Manager will provide targeted mental health case management services to adults who meet Minnesota State Statute requirements for serious and persistent mental illness (SPMI) and children who meet Minnesota State Statute requirements for severe emotional disturbance (SED) gain access to needed medical, social, educational, vocational, financial, and other necessary services as they relate to the recipient's mental health needs.

Mental Health Practitioners (MHP) provide home and community-based skills services to children, adults, and families with mental health concerns to improve their basic functioning under the license of a qualified mental health professional (QMHP).

Supervisory Responsibilities
Case Manager/Mental Health Practitioners do not provide direct supervision to other employees. They are expected to collaborate and coordinate with Fernbrook team members who share clients or caseload responsibilities

Duties And Responsibilities

  • Provide mental health targeted case management services to children, adults, and families.
  • Utilize various effective strategies to respond to child/adult/family needs in a respectful and professional manner.
  • Complete documentation in alignment with established content, quality, and timeliness standards.
  • Coordinate referrals and engage in ongoing collaboration with family community support services as identified in the community support plan for the child/adult/family.
  • Meet with the child/adult/family at a frequency based on client mental health need (must be a minimum of once every 30 days) and be available to meet with the child/adult/family upon their request.
  • Actively participate in discharge planning for the child/adult and, to the extent possible, coordinate the services necessary to assure a smooth transition to home and community based services, when the child/adult is leaving a residential treatment facility, regional treatment center, correctional facility, other residential placement, or inpatient hospital for mental health services.
  • Coordinate and facilitate child/adult/family quarterly team meetings.
  • Advocate for child/adult/family to resolve crises and reduce barriers.

  • Seek supervisory guidance and consultation as needed.

  • Attend and engage in staff meetings in ways that support your professional needs.
  • Complete 30 hours of continuing education related to mental health every two years.
  • Uphold and communicate Fernbrook Family Center's mission internally and externally.
  • Follow all agency policies and procedures.
  • Case Managers have productivity standards in regard to client-contact, and are required to attend mandatory meetings, which may focus on continuing education, program development, and client consultation.
  • Meet MHP productivity requirement of clinical services with families/groups, as well as weekly billable productivity.
  • Perform other duties as assigned

Physical Demands

  • Extended periods of sitting, listening, observing, or using a computer.
  • Occasional lifting of up to 15 pounds.

Position Type and Expected Hours of Work

  • Work hours are flexible, generally between 7:00 a.m. and 7:00 p.m.
  • Regular travel to client homes, community settings, Fernbrook offices, and off-site locations where clients may be placed is required.
  • Salaried employees work 40–43 hours per week; additional hours may be required to meet client and program needs.
  • Hourly employees may work up to 40 hours per week; overtime must be pre-approved by a supervisor.

Education And Experience

  • Reliable transportation, a valid driver's license, automobile insurance, and an acceptable driving record are required for positions involving community-based client contact.
  • Must pass a criminal background check in accordance with agency and Department of Human Services requirements.
  • A case manager must have:
  • A Bachelor's degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology, or nursing from an accredited college or university; OR
  • Have three or four years' experience as a case manager associate; OR
  • Be a registered nurse without a bachelor's degree and have a combination of specialized training in psychiatry and work experience consisting of community interaction and involvement or community discharge planning in a mental health setting totaling three years; OR
  • Have completed 4,000 supervised hours providing mental health services; OR
  • Have completed 30 semester hours in behavioral sciences plus 2,000 supervised hours providing mental health services.

Preferred Education And Experience

  • Experience working with children, adults, and families.
  • Bilingual.
  • Case Management experience
  • Skilled in assessing diverse client needs
  • Knowledgeable about leveraging local community resources
