Job Overview
We are currently hiring a
Lead Personal Support Worker
to provide compassionate, one-on-one care for children and adults with developmental disabilities through the
Home-Based Support Services program (DHS)
. This role requires a high level of responsibility, flexibility, and a concierge approach to care, ensuring each individual receives exceptional support tailored to their needs.
Responsibilities
- Deliver direct care and support in client homes and community settings
- Implement and document client goals, objectives, and behavior plans
- Promote independence through support with daily living activities (ADLs), such as personal care, household tasks, recreational engagement, and community involvement
- Ensure client safety at all times—clients must never be left unattended
- Maintain accurate documentation and data collection each shift
- Attend and complete required training, including state-mandated DSP certification within 90 days
- Communicate and collaborate with the client's support team, family, and Clinical Manager
- Immediately report medical issues, behavioral concerns, incidents, and any allegations of abuse or neglect
- Assist individuals who may exhibit challenging or maladaptive behaviors
Schedule
- Flexible, variable schedule
- Morning, afternoon, and evening availability required
Work Locations:
- Naperville-based office
- Client homes throughout Will and DuPage Counties (some clients may be located outside these areas)
- Community settings and other locations as assigned
Qualifications
- 21 years of age or older
- Valid driver's license in good standing
- Access to an insured, registered vehicle for transporting clients
- Ability to work with both children and adults
- Strong communication and documentation skills
- DSP Certification (or ability to complete within 90 days)
- CPR and First Aid Certification (or ability to obtain upon hire)
- Experience with individuals with developmental disabilities or mental health diagnoses
- Experience working with individuals exhibiting maladaptive behaviors
- Must pass local and/or state background checks
