dedicated and compassionate Physical Therapists (PTs) and Occupational Therapists (OTs)

Your Role:

• Deliver personalized, high-quality therapy services to residents in our skilled nursing facility
• Evaluate, develop, and implement individualized treatment plans
• Provide ongoing physical or occupational therapy services and reassessments
• Document therapy services accurately and in compliance with all regulations
• Work closely with the interdisciplinary team to ensure the best outcomes for residents
• Promote independence, mobility, and quality of life for every resident you serve
• Follow all facility policies, procedures, and professional standards

Who We're Looking For:

• Current Physical Therapist (PT) or Occupational Therapist (OT) license in Colorado or a valid compact license
• Passionate about improving residents' quality of life through therapy
• Strong communication and documentation skills
• Experience in a skilled nursing or long-term care setting is a plus, but not required
• Collaborative team player committed to excellent care
