Job Description: MDS Coordinator (Skilled Nursing Facility)
Department: Nursing
Reports To: Director of Nursing (DON)
Position Summary: The MDS Coordinator is responsible for planning, coordinating, and managing the Minimum Data Set (MDS) process in accordance with federal and state regulations and company policy. This position plays a critical role in ensuring accurate resident assessments, optimal reimbursement, regulatory compliance, and high-quality resident care. The MDS Coordinator collaborates closely with nursing staff, therapy, billing, and interdisciplinary team members to support resident-centered care planning.
Key Responsibilities:
Assessment & Care Planning
- Complete and oversee the completion of the MDS, CAAs, and care plans for all residents per regulatory timelines.
- Ensure accuracy and completeness of all assessment data.
- Lead and coordinate interdisciplinary team meetings for care planning and resident assessment review.
- Monitor changes in resident condition and initiate appropriate assessment processes.
Compliance & Regulatory Oversight
- Ensure compliance with CMS regulations, state requirements, and facility policies regarding resident assessments.
- Maintain current knowledge of RAI guidelines, PDPM requirements, and regulatory updates.
- Participate in quality assurance and process improvement initiatives related to documentation and assessment accuracy.
Reimbursement & PDPM Management
- Accurately capture clinical conditions and functional status to support appropriate PDPM reimbursement.
- Work collaboratively with therapy, nursing, and billing departments to ensure accurate case-mix documentation and timely submission.
- Monitor assessment schedules, ARDs, and billing cycles.
Documentation & Communication
- Maintain organized and timely documentation within the EMR and other required systems.
- Communicate assessment findings, resident status updates, and care plan changes to the care team.
- Educate staff on the MDS process, documentation standards, and changes in regulations when needed.
Resident & Family Engagement
- Support person-centered care by involving residents and families in care planning.
- Address questions related to the assessment and care planning process.
Qualifications Required:
- Licensed Registered Nurse (RN)
- Experience in long-term care or skilled nursing facility.
- Strong understanding of the MDS process, RAI guidelines, and care planning.
Preferred:
- Two or more years of experience as an MDS Coordinator.
- Familiarity with PDPM requirements and reimbursement guidelines.
- Experience with the electronic medical record systems (EMR), PointClickCare

Physical Requirements
- Ability to sit, stand, and walk for extended periods.
- Ability to lift up to 50lbs as needed.
- Visual and auditory acuity sufficient to assess residents and review documentation.
