The Cape & Islands Veterans Outreach Center (CIVOC) is a nonprofit organization dedicated to providing comprehensive, life-sustaining services and programs to veterans and their families on Cape Cod and the Islands. Our mission focuses on delivering service-enriched housing, supportive programs, healthy food, counseling, case management, and referrals to promote stability and independence. CIVOC operates transitional housing programs in Dennis and Hyannis, offering veterans a pathway to sustainable living. As a recipient of Supportive Services for Veteran Families (SSVF) funding from the VA, CIVOC provides case management, outreach, housing assistance, healthcare navigation, and temporary financial support to veterans who are homeless or at risk of homelessness.
Position Summary:
The SSVF Case Manager will conduct assessments of eligibility and needs for veterans qualifying or qualified for SSVF assistance, in accordance with VA NOFO guidelines and a housing-first approach. In collaboration with the client, the position develops individualized housing stability plans aimed at preventing homelessness or achieving rapid re-housing. The role focuses on coordinating and implementing SSVF-specific supportive services, including temporary financial assistance (TFA), outreach, and referrals to promote long-term independent housing stability. The case manager will handle intakes for SSVF-eligible veterans, perform homeless prevention screenings using VA-approved threshold scores, and ensure all activities align with SSVF program standards. Documentation and tracking must adhere strictly to VA data guidelines, including entry into HMIS and other required systems, with regular follow-ups to monitor benchmarks, outcomes, and reporting requirements. The incumbent will collaborate with VA Regional Coordinators, HUD/VASH, and community partners for seamless service delivery. Most job activities are performed in the community, including at shelters, social service agencies, veteran residences, and other appropriate locations within MA-503 (Barnstable, Dukes, Nantucket Counties), requiring travel as needed through a mobile case management model. This role is distinct from general transitional housing case management and reports to the Program Manager.
Qualifications:
· Relevant education and/or experience in human services or related field. Bachelor's degree preferred, not required.
· Minimum of three years of professional experience working with veterans or at-risk populations, with a strong preference for candidates with experience in SSVF or VA-funded programs, including knowledge of VA data guidelines, HMIS, and UDPAAS.
· Valid driver's license required.
· Ability to travel and drive as needed, including to locations across Cape Cod and the Islands within the MA-503 catchment area.
· Ability to be supportive and respectful to all veterans and household members served.
· Ability to work in a variety of settings with culturally diverse families.
· Ability to be culturally sensitive and appropriate.
· Capability to work both independently and as part of a team.
· Ability to collaborate with in-house and external referral sources, including VA, HUD/VASH, and SSVF partners.
· Possess strong written and oral communication skills, with excellent documentation abilities per VA SSVF data standards.
· Proficiency in Microsoft Office suite, strong organizational skills, and familiarity with VA-required data systems (e.g., HMIS for data quality reporting, UDPAAS for grant activities).
· Demonstrate a strong level of understanding and comfort with various forms of technology, including data entry and reporting systems.
· Ability to take direction and participate in regular supervision, including compliance with SSVF monitoring and audits.
· Preference given to candidates who are veterans and/or have lived experience with homelessness or housing instability.
Responsibilities:
· Conduct SSVF eligibility and intake assessments for qualifying veterans, including homeless prevention screenings and threshold score determinations per VA guidelines.
· Assess functional and social needs of the veteran to co-create an individualized housing stabilization plan, incorporating SSVF services such as TFA, rapid re-housing, or shallow subsidy as applicable.
· Develop and maintain relationships with community service providers, agencies, and VA partners (e.g., Regional Coordinators) to support SSVF goals.
· Determine and access appropriate Veterans Administration, community, state, and federal benefits and resources, ensuring coordination with HUD/VASH and other TA providers.
· Provide SSVF-focused case management and follow-up services through a lens of progressive engagement, tracking client progress with required VA follow-ups (e.g., 30/60/90-day intervals) to monitor housing stability outcomes.
· Coordinate referrals and follow up on outcomes, ensuring alignment with SSVF benchmarks and reporting.
· Maintain client confidentiality in accordance with VA and agency standards.
· Provide support services in crisis situations appropriately and quickly, escalating to VA resources as needed.
· Assist veterans in obtaining and maintaining permanent housing, including support with housing applications, landlord engagement, and SSVF-specific assistance.
· Document and maintain accurate information in electronic client files and VA-required systems (e.g., HMIS, UDPAAS), including progress notes on all visits, contacts, and outcomes, on a regular basis per SSVF data guidelines.
· Ensure compliance with VA SSVF data quality standards, including timely reporting, benchmarks, and preparation for audits (e.g., FOFA).
· Knowledge and understanding of SSVF program guidelines, NOFO requirements, and company policies.
· Adherent to agency policy, procedures, professional code of ethics, and all SSVF compliance requirements.
· Complete various tasks assigned by the Executive Director.
· And other duties assigned by the Program Manager, including participation in SSVF webinars, trainings, and regional meetings.
Job Type: Full-time
Pay: $55, $58,000.00 per year
