Position Summary

The Program Manager for Short-Term Post-Hospitalization Housing (STPHH) and Recuperative Care oversees MACC's four-county transitional housing program for neighbors recently discharged from hospitals, SNFs (Skilled Nursing Facilities), and other post-acute settings.

This role requires a clinically skilled Licensed Vocational Nurse (LVN) with experience in post-acute environments who can ensure program safety, quality, CalAIM compliance, and smooth transitions of care. The Program Manager provides leadership for Community Health Workers/Care Coordinators assigned to each housing site, supervises day-to-day operations of STPHH homes, collaborates with hospitals and discharge planners, and oversees program standards and documentation.

The Program Manager will be responsible for admissions coordination, staff oversight, CalAIM encounter documentation, compliance monitoring, and community partner relationships. Regular travel across Butte, Colusa, Tehama, and Yuba/Sutter Counties is required; a program vehicle is provided.

Key Responsibilities

Program Operations & Oversight

  • Oversee the operations of STPHH and Recuperative Care homes across four counties, ensuring safe, clean, and supportive housing environments for recently discharged neighbors.
  • Set daily and weekly workflows for Community Health Workers/Care Coordinators, including intake, wellness checks, care coordination, and discharge planning.
  • Maintain strong relationships with hospitals, SNFs, discharge planners, social workers, and partner agencies to facilitate smooth admissions and transitions of care.
  • Coordinate with the Director of Community Development to ensure alignment with MACC's mission, goals, and county-level expectations.

Clinical Coordination (Non-Direct Care)

  • Use LVN experience to evaluate the appropriateness of referrals, review discharge summaries, and identify potential risks or needs.
  • Provide clinical oversight and guidance to non-clinical staff, including verifying that a neighbor's housing placement is safe based on nursing judgment and post-acute standards.
  • Liaise with external medical providers, hospital case managers, and SNF staff to clarify clinical instructions relevant to safe recuperation.
  • Ensure that care coordination follows standard protocols for observation, symptom escalation, and return-to-care pathways.

Admissions & Transitions of Care

  • Review all referrals for eligibility under CalAIM STPHH and Recuperative Care criteria.
  • Assess whether prospective participants are appropriate for the program based on safety, mobility, nursing considerations, and environmental fit.
  • Coordinate admission logistics, arrange transportation when needed, and ensure all intake documentation is complete.
  • Oversee discharge planning, including follow-up appointments, care coordination, community resource connections, and safe housing exit plans.

Staff Supervision & Leadership

  • Directly supervise Community Health Workers and/or Care Coordinators assigned to each STPHH home.
  • Provide coaching, performance management, scheduling support, and problem-solving for the team.
  • Conduct trainings on CalAIM documentation, trauma-informed service delivery, safety protocols, and housing standards.
  • Lead regular team meetings, site visits, and operational audits across all program locations.

CalAIM Compliance & Documentation

  • Maintain full compliance with the Enhanced Care Management (ECM), Community Supports, and STPHH/Recuperative Care documentation requirements.
  • Monitor encounter submissions, service notes, eligibility verification, TARs, and authorization protocols.
  • Train staff on accuracy, timeliness, and quality of documentation within MACC's EHR/HIM workflows.
  • Prepare reports and metrics for internal leadership and managed care plans as required.

On-Call & Emergency Response

  • Participate in program on-call rotation, responding to urgent issues, neighbor concerns, and operational needs during evenings or weekends.
  • Provide guidance for crisis response, escalated care needs, or coordination with emergency services when necessary.

Community Engagement & Faith Alignment

  • Represent MACC in meetings with hospitals, county partners, CBOs, and other agencies.
  • Ensure all program activities reflect MACC's faith-based mission and values, offering compassionate, respectful, neighbor-centered support.
  • Participate in prayer, devotional moments, and faith-aligned cultural practices as part of the leadership team.

Qualifications

  • Current California LVN license in good standing.
  • Minimum 3–5 years of experience in Skilled Nursing Facilities, post-acute care settings, rehabilitation centers, or transitional care programs.
  • Experience supervising staff or managing care teams preferred.
  • Knowledge of CalAIM Community Supports, Enhanced Care Management, or similar medical/social care models strongly preferred.
  • Strong understanding of discharge planning, transitions of care, and post-acute safety considerations.
  • Ability to travel regularly across Colusa, Butte, Yuba/Sutter, and Tehama Counties (program vehicle provided).
  • Strong communication, leadership, and multitasking skills.
  • Comfort working in a faith-forward organizational culture and alignment with MACC's mission and values.

Work Environment

This role requires frequent travel to STPHH and Recuperative Care homes and partner facilities; a program vehicle is provided. Evening or weekend availability may be required during on-call rotations. The Program Manager may work partially remotely when not performing field-based duties.
