Job Summary:

Title: Licensed Practical Nurse - Med/Surg

Location: Coxsackie, NY 12051

Duration: 2 years

Shift: Variable hours hours a week.

Pay rate: Lucrative pay + Benefits + Bonus

Requirements:

  • Must have 2+ years of recent experience.
  • Active license in NY state.
  • COVID Vaccinated.
  • BLS / CPR Preferred.

Responsibilities :

  • Perform comprehensive patient assessments by taking vital signs, conducting physical examinations, and evaluating physiological data to inform care plans.
  • Administer medications and injections accurately following physician orders, including IV infusions and injections with sterile technique.
  • Manage complex clinical tasks such as catheterization, tube feeding, wound suturing, and ventilator management in accordance with best practices.
  • Utilize EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth to document patient care activities thoroughly and accurately.
  • Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or memory impairments through tailored memory care protocols.
  • Assist with diagnostic evaluations including specimen collection/processing, medical imaging coordination, and performing laboratory tests like phlebotomy and venipuncture.
  • Support acute pain management strategies and participate in triage processes to prioritize urgent patient needs effectively.
  • Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care to ensure seamless transitions across care settings.
  • Implement infection control procedures adhering to HIPAA regulations and best practices for sterile processing.
  • Educate patients and families on health coaching topics such as nutrition, medication adherence, chronic disease management, and family planning when appropriate.
  • Respond swiftly to emergencies in hospital environments such as Level I Trauma Centers or ICU units by managing airway issues, ventilators, or cardiac emergencies.
  • Maintain detailed medical documentation ensuring compliance with ICD coding standards (ICD-9/ICD-10) for accurate billing and record keeping.

Qualifications :

  • Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice with current CPR/BLS certification.
  • Proven experience in diverse clinical environments including hospital medicine, long-term care facilities, assisted living communities, or home & community care settings.
  • Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques relevant to acute and chronic conditions.
  • Experience working with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks is highly preferred.
  • Skilled in patient assessment techniques including vital signs measurement and physical examinations; capable of performing venipuncture and administering injections confidently.
  • Familiarity with specialized areas such as dementia care (Alzheimer's), memory support programs, hospice care protocols, or pediatric nursing is advantageous.
  • Ability to work effectively with people of all ages—including children with neonatal ventilators or toddlers—and those with developmental disabilities or complex health needs.
  • Excellent communication skills combined with critical thinking abilities to triage urgent situations efficiently while maintaining compassionate patient interactions.
