Job Overview
We are seeking a compassionate and experienced Case Manager to support individuals and families through various social services. The ideal candidate will possess strong skills in crisis intervention, addiction counseling, and behavioral health, providing critical support to clients navigating complex challenges. This role involves coordinating care, advocating for clients, and connecting them with community resources such as Child Welfare, Section 8 housing, Medicare, and workforce development programs. Duties

  • Conduct comprehensive assessments to identify client needs in areas such as crisis management, addiction recovery, child welfare, and housing assistance.
  • Develop individualized care plans tailored to each client's circumstances and goals.
  • Provide crisis intervention and behavioral health support, utilizing motivational interviewing techniques to motivate positive change.
  • Coordinate services across multiple agencies including Medicaid, Section 8 housing programs, and workforce development initiatives.
  • Offer supervision and mentoring to clients, including those involved in childcare or parenting programs.
  • Monitor client progress regularly, adjusting care plans as necessary to ensure objectives are met.
  • Advocate on behalf of clients to secure resources and navigate complex systems such as Medicare or child welfare services.
  • Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with organizational policies.
  • Collaborate with team members and community partners to ensure holistic support for clients.

Experience

  • Proven experience in social work, behavioral health, or related fields with a focus on crisis intervention and management.
  • Prior supervisory or mentoring experience is highly desirable.
  • Knowledge of addiction counseling techniques and experience working with individuals facing substance use challenges.
  • Familiarity with child welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives.
  • Strong skills in motivational interviewing and crisis management strategies.
  • Experience working with diverse populations including children involved in childcare or foster care settings is preferred.
  • Excellent communication skills with the ability to build trust and rapport with clients from various backgrounds. This position is integral to supporting vulnerable populations through compassionate care and resource coordination. Pay: $ $29.40 per hour
