About the Role

As a Case Manager, you will play a key role in empowering up to 30 participants as they pursue their personal and employment goals. You'll coordinate individualized support, build strong relationships with families and service coordinators, and ensure participants feel seen, supported, and motivated every step of the way.

This temp-to-hire position is ideal for someone who is ready to grow, lead, and inspire—both in the community and in the workplace.

Training & Onboarding

We invest in your success from day one.

Your first two months include:

  • Hands-on field training as a Job Coach

  • Comprehensive learning in policies, documentation, and case management

  • Coaching and support to help you confidently manage your own caseload

You'll receive the tools and guidance needed to become a strong, independent Case Manager, and a valued member of our team.

Responsibilities

  • Develop, monitor, and update Individual Service Plans (ISPs)

  • Conduct meetings with participants, families, and service coordinators

  • Maintain accurate and detailed documentation and progress reports

  • Support, train, and mentor new staff members

  • Work collaboratively with management and program staff to ensure participant success

Qualifications

  • Bachelor's degree in Psychology, Social Work, Human Services, or related field required

  • Minimum 2 years of experience working with adults with developmental or intellectual disabilities

  • Strong communication, organizational, and interpersonal skills

  • Knowledge of person-centered planning and behavioral strategies

  • Valid driver's license and reliable transportation
