Pediatric RN Case Manager- Office Position

**Job Overview:**

We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, providing medical support, and ensuring the highest quality of home healthcare services.

**Responsibilities:**

• Conduct patient assessments and develop individualized care plans

• Coordinate with healthcare providers to ensure seamless patient care delivery

• Monitor and evaluate patient progress and update treatment plans as needed

• Provide education to patients and families on healthcare management

• Collaborate with interdisciplinary teams to optimize patient outcomes

• Maintain accurate medical records and documentation

• Participate in discharge planning activities

**Qualifications:**

• Valid RN license in the state of Minnesota

• 1 year of home health care experience. Complex pediatric home care experience is a plus.

-1 year of RN Case Management experience. Complex pediatric home care experience is a plus.

• Knowledge of managed care principles and practices

• Ability to assess vital signs and provide acute/critical care interventions

• Familiarity with medical records systems and discharge planning processes
