Join Our Team
The Case Manager is responsible for performing client care activities for clients across our continuum of care including detox, residential and partial hospitalization treatment programs. The Case Manager provides comprehensive, person-centered support to clients engaged in substance use and behavioral health services. This role coordinates care, ensures continuity across providers, and assists clients in achieving treatment goals through advocacy, resource linkage, and ongoing monitoring. The Case Manager maintains a strengths-based, recovery-oriented approach in all interactions.

Case Manager Requirements
- Substance Use Disorder: 1 year (Preferred)
License/Certification:
- Required: bachelors degree in a related field, CADC, or Clinical License in ME (LADC, CADC, LCSW, LCPC, LMHC, LMFT)
- Fully or Conditionally Licensed substance use or mental health license may also apply
Case Manager Responsibilities
- Actively participates in Interdisciplinary Treatment Plan meetings.
- Shares pertinent information with team as obtained from clients family, or others as indicated throughout program.
- Attends daily and weekly staffing meetings per schedule.
- Evaluates the impact of therapeutic interventions upon the therapeutic milieu.
- Facilitates group therapy.
- Performs needs assessments.
- Documents the assessment of client progress/regress and interventions provided in the medical record, on an ongoing basis.
- Recognizes, responds to client care needs to facilitate successful completion of treatment.
- Support clients in navigating benefits such as MaineCare, SNAP, FMLA, and disability services.
- Communicates effectively with client, staff and physicians.
- Attends and participates in monthly Program Staff meeting as scheduled.
- Adheres to policies and procedures.
Job Type: Full-time
Pay: $55, $65,000.00 per year
