Essential Duties & Responsibilities of Social Worker:
- Meet all physical, sensory, and environmental requirements of the role.
- Participate in process-improvement initiatives that enhance resident satisfaction, workflow efficiency, and the overall work environment.
- Provide leadership for the Social Services department, including hiring, training, coaching, developing, and – when necessary – conducting corrective action or terminations in a fair and compliant manner.
- Ensure all employee actions and disciplinary processes comply with federal, state, and local laws and are administered without discrimination.
- Complete all Social Services components of the MDS, RAPs, Care Plans, Progress Notes, and required assessments accurately and on time.
- Support residents and families with social, emotional, psychosocial, and adjustment-related needs.
- Participate in interdisciplinary care planning to identify and address residents' social and emotional needs.
- Maintain detailed, regulatory-compliant progress notes reflecting each resident's adjustment, care progress, and responses to interventions.
- Make referrals to appropriate community agencies and keep accurate records of all referrals.
- Assist in discharge planning and help prepare residents for successful transitions.
- Serve as a resource during the orientation process for new residents and families.
- Act as a central information source for residents, families, and the public.
- Visit residents regularly and assist with communication, resource coordination, or needs involving families, friends, or legal representatives.
- Coordinate logistics, communication, and content for monthly Family Council meetings.
- Manage and coordinate room changes to ensure smooth transitions for residents.
- Contact residents or legal representatives to schedule and encourage participation in care plan meetings.
- Maintain consistent communication with residents, legal representatives, and family members, providing updates and involving them in ongoing care.
- Collaborate with the interdisciplinary team to meet the needs of residents with limited or no family support.
- Foster positive working relationships with leadership, medical staff, physicians, consultants, other departments, residents, families, volunteers, and external agencies.
- Work closely with the facility's mental health provider to ensure psychosocial needs are identified and addressed promptly.
- Assist in educating the community on aging, resident rights, and facility services.
- Help new residents adjust to the long-term care environment by providing orientation and ongoing support.
- Perform other duties and special projects as assigned.
Job Requirements:
- Bachelor's degree in social work, or bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology; AND One (1) year of supervised social work experience in a health care setting working directly with individuals
- Effective verbal and written English communication skills.
- Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
- Highest level of professionalism with the ability to maintain confidentiality.
- Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
- Customer service oriented with the ability to work well under pressure.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
- Strong analytical and problem-solving skills.
- Ability to work with minimal supervision, take initiative and make independent decisions.
- Ability to deal with new tasks without the benefit of written procedures.
- Approachable, flexible and adaptable to change.
- Function independently, and have flexibility, personal integrity, and the ability to work effectively with stakeholders and vendors.
