This PA-C role is ideal for a clinician who enjoys *autonomy, hands-on care, and meaningful patient relationships*. You’ll serve as a consistent medical presence within assigned communities, combining primary care with wound management in a setting that values time, trust, and teamwork.
*Your Role.*
* Provide on-site medical assessments, primary care, and wound management.
* Manage chronic conditions and preventive care for senior patients.
* Round consistently within assigned communities to build continuity and trust.
* Collaborate closely with nursing staff, therapy teams, and families.
* Ensure timely, thorough documentation and follow-up care.

* Active Minnesota PA-C license.
