Facility: Woodsedge Location: Bemidji, MN Address: 1000 Anne St NW, Bemidji, MN 56601, USA Shift: Day Job Schedule: Full time Weekly Hours: 40.00 Starting Rate: $25.12 - $33.45 Union Position: Yes Department Details Interdisciplinary LTC setting; experienced therapy team in Scenic northern MN Will provide therapy in LTC setting in Bemidji and Blackduck MN This position is not remote Job Summary Assists patients with therapy activities and exercises scheduled in a treatment plan, under the supervision of a physical therapist, to improve patients' quality of life and ability to perform daily activities. Provides, monitors and records therapy services to patients with physical, cognitive, developmental impairments, in line with related standard concepts, practices and procedures. The physical therapy assistant selects, implements, and makes modifications to therapeutic activities and interventions that are consistent with demonstrated competency levels, patient's goals, and the requirements of the practice setting and/or state licensure. Responsible for being knowledgeable about the patient's targeted physical therapy outcomes and for providing information and documentation related to outcome achievement. Primary location providing therapy in LTC setting in Bemidji and Blackduck MN Qualifications High school graduate is preferred. Must be physically capable to assist with patient transfers, activities of daily living (ADLs), and gait training. Licensure as a physical therapist assistant in the state of Minnesota. Affiliation with the American Physical Therapy Association by registration, certification, and/or membership is recommended.
