The Home Health Physical Therapist provides in-home therapeutic care to patients recovering from injury, surgery, or illness, helping them regain movement and function. The PT plays a critical role in evaluating patients, developing individualized therapy plans, and working collaboratively with the healthcare team. The role requires excellent clinical judgment, patient education skills, and a compassionate approach to care.
*Minimum Qualifications *
* Doctorate or master’s degree in physical therapy from an accredited institution.
* Current Physical Therapist license in the state.
* Minimum of 2 years of experience in physical therapy, with at least 1 year in home health or a related setting.
* Proficient in developing and implementing care plans for diverse patient needs.
* Ability to work independently and manage time efficiently.
* Proficient in electronic health records (EHR) systems.
* Strong communication and interpersonal skills.
* Current CPR certification.
*Job Responsibilities *
* Conduct comprehensive patient assessments to determine therapy needs and develop individualized care plans.
* Administer therapeutic interventions such as exercises, stretches, and manual therapy techniques to improve patient mobility and function.
* Educate patients and caregivers on exercises and techniques to continue therapy at home.
* Monitor and document patient progress, adjusting care plans as needed.
* Collaborate with the nursing and therapy teams to ensure coordinated, comprehensive care.
* Assist patients in selecting and using assistive devices such as walkers, canes, or wheelchairs.
* Provide recommendations for home modifications to ensure patient safety and accessibility.
* Maintain accurate and timely documentation in electronic health records.
* Ensure compliance with home health regulations and company policies.
* Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.
* Perform any additional duties as assigned to meet the needs of the department and organization.
*Reporting Relationship *
* Reports to: Director of Rehabilitation or Director of Clinical Services
* Supervises: Physical Therapy Assistants and Home Health Aides (as related to therapy tasks) •
*• Internal Relationships:*
Works closely with: • Nursing team (for patient care coordination)
* Occupational therapists, speech therapists, and other therapy disciplines (for comprehensive patient care)
* Care management team (for discharge planning and follow-up care)
*External Relationships: *
Liaises with:
* Patients and their families (for patient education and care planning)
* Physicians (for therapy orders and updates)
* Equipment vendors (for patient assistive devices)
*Physical Requirements *
* Manual Dexterity: Ability to use physical therapy equipment and perform hands-on patient care.
* Mobility: Ability to move between patients' homes, including standing, walking, bending, and assisting patients with exercises.
* Lifting: Ability to lift or move objects and patients, typically up to 50 lbs.
