Über
We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, developing individualized care plans, and implementing effective physiotherapy interventions. The Physical Therapist plans and administers physical therapy treatment programs for Kinship PACE participants to maintain and/or restore function, relieve pain, and prevent further disability resulting from disease, injury, or loss of body parts.
*Duties*
* Completes assessments, progress notes, treatment or revisit notes, discharge summaries, and interdisciplinary notes according to departmental policy and procedure.
* Performs participant evaluations and treatment at the PACE site and/or in the field (participant’s home, nursing homes, etc.). Provides treatment as indicated.
* Develop specialized programs according to participants’ needs.
* Participates in developing participant care plans with other interdisciplinary team members and participates in care plan meetings in accordance with state and federal regulations.
* Assesses the need for adaptive equipment and durable medical equipment and orders and facilitates authorization of such equipment.
* Documents all pertinent participant information as required, including, but not limited to, assessments, progress notes, and care plans according to departmental policy and procedure.
* Develops, instructs, and supervises other staff and/or family members on therapy programs to be carried out by individuals other than a physical therapist.
* Initiates discharge planning in a time-appropriate fashion and orders equipment for discharge in collaboration with team members.
* Participates in delivery of in-service training programs for site and departmental staff.
*Skills*
* Doctor of Physical Therapy Degree (DPT) required from an APTA-accredited university or equivalent.
* Valid State licensure as a Registered Physical Therapist (DPT)
* One (1) year of experience as a Physical Therapist and/or experience with the Geriatric population required.
* Ability to travel to participant homes, nursing homes, and sites/clinics.
* Valid driver's license, current insurance.
* Knowledge of PACE regulations is preferred.
